Foster Care Case Manager- ACI 

Description

Position Description: The Foster Care Case Manager provides intensive, home and community based, skill building and case management services to seriously emotionally and behaviorally troubled (SED) youth and their families. As the Case Manager, you will maintain a caseload of youth and families and caseload size may vary based upon the needs of assigned families. You will engage clients and families as partners in the care planning process and operates from a strengths perspective.

Responsibilities Include:

  • Interview clients to obtain information pertinent to establishing and maintaining effective client records and service plans. Gather information in a timely and systemic manner that will enable ongoing diagnosis, assessment, and treatment planning.
  • Demonstrate the ability to develop and maintain strengths-focused, professional alignment and engagement with families.
  • Develop focused, measurable and achievable service plans that are consistent with the family's identified needs and cultural assessment.
  • Assist and advocates on behalf of clients and families to secure needed resources, services and supports.
  • Collaborate with families and clients in the development of crisis plans and provides crisis assistance and coaching as needed.
  • Assist clients and families with the development of skills that will enhance and improve functioning in areas that include but are not limited to interpersonal relationships, anger management, coping, etc.
  • Assist clients and families with monitoring and managing symptoms related to mental, emotional and behavioral symptomatology.
  • Coordinate the delivery of services identified on client treatment plans (ISP) and provides case management.
  • Form and maintain collaborative working relationships with natural supports, collateral professionals and/or agencies concerning individual cases.
  • Secure supervisory signatures per licensing guidelines and agency policy/procedure.
  • All required training, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards
  • Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards. All documentation to be submitted within set program deadlines.

Requirements:

  • Minimum Bachelor's Degree in social work, counseling or psychology required, Master's preferred.
  • Valid State of Ohio LSW or LPC
  • A valid drivers' license, personal transportation and insurance.
