We believe vaccination is the best protection for our patients, community and Visiting Nurse staff.   Location Community Care   Category Community Care   Description Provides enhanced care management activities necessary to meet the needs of Members assigned to the Multi-Team. This position is responsible for the development and implementation of individualized Carepaths through brokering and coordinating services; and the monitoring and evaluation of all clinical outcomes to ensure that services are provided in a professional, comprehensive and cost-effective manner.   Full-Time/Part-Time Full-Time   Position Requirements Education: Master’s Degree in Social Work, Sociology, Psychology or a related field required Licensure/Certifications: Professional licensure preferred, but not required. Current CPR certification required Previous Experience: 2 years experience in case management in a health care field. Preference given to national certification in case management. Knowledge/Training: Knowledge of Medicare, Medicaid and other funding source regulations as applied to service program.
