Description

JOB SUMMARY: Provide consumer diagnostic assessments and admission services for Partial Care Program.

DUTIES AND RESPONSIBILITIES:

  1. Orient potential consumers to Catholic Charities Partial Care Services.
  2. Conduct diagnostic and level of care assessments.
  3. Complete admissions paperwork including authorization and initial service plan during the intake process.
  4. Complete a written comprehensive assessment for each consumer during the intake process. The written comprehensive assessment will be completed within one month after acceptance to the program and prior to development of the individualized recovery plan.
  5. Write progress notes once per week as per DMH guidelines. This includes the following: completions of the daily rating scale with consumer self-report, psychosocial assessment which will be completed within the first six weeks of admission; Individualized Recovery Plan will be formulated and implemented at the completions of the comprehensive assessment but no later than six weeks from admission; IRP will be revised and revised within three months of its development, every three months for the first year, and every six months thereafter; weekly notes summarizing progress, response to at least one specific treatment intervention in the IRP, participation in PC activities, and significant events will be completed by the end of the following week.
  6. Develops realistic service plans based on target/problem areas with consumer’s and collaborate achievable, measurable objectives.
  7. Meets individually with consumer’s weekly to monitor progress toward identified objectives until transferred to new partial care counselor during intake process.
  8. Collaborates with medical staff to ensure a seamless system of care; in addition collaborating with staff and consumer in determining the most appropriate treatment program.
  9. Present all new case information to treatment team at weekly staff meetings.
  10. Completes weekly progress notes and documents all direct and indirect activity on behalf of consumers.
  11. Acts as primary liaison to the State Hospital and Intensive Case Management Services and arranges for brief visitors; In addition to Mercer County Board of Social Services and The Community Law Project.
  12. Establish and maintain working relationships/communication with all referral sources, including hospitals, other agencies, physicians, and treatment providers.
  13. Consults with other service providers and documents their participation in the consumer’s recovery process.
  14. Supervises case managers who support engagement of new and high risk consumers.
  15. Functions as case manager for new consumers until consumer is assigned to a primary partial care counselor.
  16. Links consumers to appropriate entitlement sources, including General Assistance, Medicaid, Food Stamps, Housing, Medical, and Food Banks.
  17. Identifies and accesses community resources to support group activities.
  18. Compiles and reports on referrals, admission, and retention data monthly.
  19. Maintain established productivity key performance indicators.
  20. Maintains accurate and up to date case records consistent with agency and funding source requirements.
  21. Completes weekly progress notes and documents all direct and indirect activity on behalf of consumers.
  22. Involves and engages family and significant others in the recovery process and assist them in understanding the illness, coping with it and managing its effects.
  23. Monitors and utilizes supervision and training as an opportunity for enhancing professional competence.
  24. Participates in Quality Improvement activities, evaluating program effectiveness and outcomes.
  25. Provide individual, family, and group counseling to consumers as needed.
  26. Perform other duties as assigned.

 
Position Requirements

QUALIFICATIONS:

  • Master’s in Social Work, Psychology or Counseling, with a LCSW or LCADC or licensed eligible.
  • Minimum 1 year of clinical experience in a psychiatric setting.
  • Ability to relate to a variety of consumers with flexibility and good judgment.
  • Ability to assertively present program to potential consumers.
  • Strong organizational and documentation skills.
