SAFY of Cleveland is seeking a mission focused professional to serve as a part time Bridges Liaison responsible for case management functions of a caseload for young adults in the Bridges program.

As the Bridges Liaison you would:

  • Help young adults develop a plan and set goals.
  • Meet with young adult’s face to face at least once a month.
  • Review plan with young adults every 90 days to acknowledge their achievements and plan their next steps.
  • Help the young adults maintain program eligibility if their circumstances change.
  • Help the young adults identify individuals and resources that can be permanent connections even after they've completed the program.
  • Teach young adults’ everyday skills, like money management, how to do laundry, how to grocery shop, etc.
  • Help with college or a vocational program enrollment.
  • Assist young adults with securing/maintaining employment.
  • Help young adults find a doctor or access other physical or mental health care.
  • Connect young adults with community resources to help them become more self-sufficient.
  • Provide guidance and be a positive adult supporter.
Position Requirements

What you bring to this role;

  • Completion of a post-secondary program (may be an associate degree or related certification) in social work, human services, behavioral health or related field is required.
  • Two years of relevant experience working with families and/or children in the child welfare system; or possess a bachelor’s degree in social work, human services, behavioral health or a related field.
