Description

As a Clinical Pharmacist you will provide an array of direct-member services ranging from addressing drug-related questions to comprehensive medication management services, identifying, preventing and resolving medication-related problems, with consultative partnering with providers nationwide. You will serve as a “Pharmacist Advocate” for our members and help them maneuver the system through information or on their behalf.

You will educate members on their medications and how to modify habits that will result in optimized results and improved health outcomes. You will connect and advise Providers on areas of concern and re-engage them on the participants medication-related health.

These services are designed to improve the well-being of our members, empower them with information to get better results from the health care system and take care of their own lives, as well as optimize medication utilization at the individual level. MOBE Pharmacists provide their services to individuals with complex patterns of health care utilization, and often have multiple chronic conditions and combinations of drug use that may significantly impair their quality of life. Members are identified via MOBE’s customers, which may include health plans, private employers, and ACOs.

Responsibilities:

  • The Clinical Pharmacist will provide direct (telephonic) and indirect member services as part of a broader program focused on empowering members through self-management principles
  • MOBE Pharmacists assess and make recommendations to optimize individual’s medication therapy, and provide personalized education and information, through multi-media and telephonic interactions with MOBE members and their providers, as relevant. The Pharmacist will use motivational interviewing techniques to promote self-management principles and empower members to achieve optimal medication-related outcomes in conjunction with their health care providers.
  • The Pharmacist must stay abreast of clinical guidelines and research developments for drug and dietary supplement/vitamin use
  • MOBE members are located nationwide, and as a MOBE Pharmacist you may be responsible for obtaining and maintaining good standing with multi-state licensures
  • The position may have opportunities to expand into data analysis, program, process, and systems development, medical writing and clinical educational content development and training

 
Position Requirements

Qualifications:

  • Candidates must possess a pharmacy degree from an ACPE-accredited institution and have experience providing ambulatory pharmacy care, direct patient medication management experience; PGY1 residency or equivalent (3+ years) direct patient care experience required. PGY2 residency in ambulatory care or psychiatry experience preferred, but not required. Board Certified in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), or Psychiatry (BCPP) preferred, but not required
  • Pharmacy licensure, or eligibility, in MN, or other state required. Willingness to obtain and maintain specific multi-state licensure within 6 months of hire, depending on business need, required
  • Candidates must have a high level of ambulatory practice skills, enthusiasm for patient care, and the interpersonal skills needed to interact effectively with health professionals, patients, and other colleagues via telephonic interactions.
  • The candidate must have a positive attitude toward the profession of clinical pharmacy and its role/potential within the broader health professions
  • The candidate must have good written and verbal communication skills. The candidate must be adept in problem-solving and analytical skills
  • The candidate must be a good listener and display empathy for medication-related needs and understand the importance of communicating in such a manner in member interactions
  • The candidate must be proficient in MS Office suite, and be capable of learning and implementing use of new technologies/documentation tools
  • The candidate must be willing to seek and maintain licensure in multiple states
