We believe vaccination is the best protection for our patients, community and Visiting Nurse staff.   Location East Branch   Category Home Health   Description The Home Health Medical Social Worker provides social, emotional and psychological services/counseling to patients and their families, enabling the patient to maintain independence and remain at home. The Home Health Medical Social Worker will facilitate alternative care placement requested by patient/family to an appropriate facility.   Full-Time/Part-Time Part-Time   Position Requirements Education: Master’s degree in Social Work from an accredited school of higher learning. Licensure/Certifications: Current licensure in the state of Georgia. LCSW preferred.  Must possess a current valid Driver’s license. Previous Experience:  Previous home health experience preferred.  At least 1 or more years of social work experience required. Knowledge/Training: Knowledge of eligibility criteria for federal and state services such as Medicare, Medicaid, AHS, SSI, Welfare, Veteran s benefits, etc.; knowledge of community resources and facilities; knowledge of counseling techniques. Computer Skills: Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
