About the Organization Hill Country Mental Health and Intellectual and Developmental Disabilities Centers is a unit of local government and a not-for-profit, 501C provider of Mental Health and Intellectual Developmental Disabilities, Substance Abuse, and Veteran Peer Services throughout the greater Texas Hill Country Region. Hill Country MHDD Centers is designated to be the Mental Health Authority by the Texas Department of State Health Services (DSHS) and the Intellectual and Developmental Disability Authority by the Department of Aging and Disability Services (DADS) for a 19 County Service Area including: Bandera, Blanco, Comal, Edwards, Gillespie, Hays, Kendall, Kerr, Kimble, Kinney, Llano, Mason, Medina, Menard, Real, Schleicher, Sutton, Uvalde and Val Verde Counties. With a Mission of Promoting Independence, Community Integration and Recovery, HCMHDD Centers operates (10) ten Mental Health Clinics, (8) Developmental Disability Centers and serving a population of over 630,349 consumers within 22,593 square miles.

Job Title: IDD Service Coordinator

Essential Responsibilities:

Hill Country Mental Health and Developmental Disabilities Centers strongly believes that every individual deserves the opportunity to achieve their fullest potential. At the present time, we are looking for a qualified candidate to fill the role of IDD Service Coordinator. The right candidate will be responsible for assessing, evaluating and identifying needs and goals of individuals with intellectual and developmental disabilities relating to personal outcomes, including recommendations for interventions and support services.

The IDD Service Coordinator is responsible for making contacts with assigned individuals in accordance with person-centered plan, and accurate and timely documentation which supports outcome-based services. All services are delivered in compliance with Health and Human Services (HHSC), the Service Coordination Rule (Chapter 412, Subchapter L.), Home and Community Based Services (Chapter 9, Subchapter D), the Texas Home Living Program (Chapter 419, Subchapter N.), and PASRR RULES: TAC, Title 40, Part 1, Chapter 19, Subchapter BB

Position Requirements

Academic / Work Experience Requirements:

  • Bachelor’s or advanced degree from an accredited college with a major in a social, behavioral or human service field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education and criminal justice.
  • One year of experience with case management or service coordination for individuals with IDD – Preferred
  • Experience with enrollment procedures for the HCS Program - Preferred
  • Bilingual (English/Spanish) Speaking - Preferred

License/Certifications Required:

  • Valid Texas Driver’s License
  • Current Automobile Insurance

Compensation $15.86 per hour / $33,000.00 Annual
