What You Would Be Doing

and case management services to children and families. Services may be provided on an individual, group, or family basis in the child's school or home environment.

The clinician/social worker reports to the clinical manager and is responsible for ensuring that the emotional and social needs of the clients are met and maintained on an individualized basis in accordance with policies and procedures, and current federal, state, and local standards, guidelines, and regulations.

Essential Functions:

  • Develops a comprehensive social history and psychosocial assessment that includes the client's problems, strengths, and preferences; including cultural, religious, and ethnic background and implications for the care plan.
  • Helps clients and families identify needs and develop a service plan to meet those needs.
  • Provide therapy and case management services to consumers targeted individualized treatment needs.
  • Orients clients and families to the program, its services, its service limitations, and clients' rights.
  • Involves clients and families in treatment planning, encouraging their attendance at care conferences or with outside providers/professionals.
  • Attends scheduled care conferences and identifies and interprets psychosocial needs of clients for inclusion in the care plan.
  • Completes mandatory documentation within timelines established by applicable regulations and policies.
  • Mediates issues that arise among clients, families, and staff.
  • Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, etc.).
  • Provides clients and families with crisis management services as needed.
  • Develops a comprehensive discharge plan in collaboration with clients, clients' families, multidisciplinary staff, and referral agencies.
  • Trains and consults with staff regarding psychosocial needs that encompass mental health, development, and diversity of individual clients to increase staff knowledge and skills.
  • Educates staff about any relevant state legislation or regulations relating to behavioral health services.
  • Advocates on a case, policy, and program level.
  • Meet required productivity requirements of 20 billable hours per week.
  • Provide clinical review and audit of clients' charts and documentation to ensure compliance and support quality improvement.
 
Position Requirements

Required Education and Experience

    • Bachelor’s or Master’s degree in social work or related field.
    • Licensed through the State of Ohio as an LSW/LPC/LISW/LPCC.

Preferred Education and Experience

    • One to two (1-2) years of social work experience in a school or community setting, working directly with children and families.
    • Experience working with people with autism spectrum disorders and developmental disabilities.
    • Experience working with people with mental health conditions.
