Clinical Case Manager (Bilingual English/Spanish)

Position Summary: This position has primary responsibility for providing clinical case management services to promote safe and stable families in West Contra Costa County. Services are preventative and include mental health and case management support delivered in community, school or office locations. Services may also include individual youth support and can comprise home visits. All services are delivered using our unique methodology that integrates restorative practices with evidence-based mental health treatment.

Essential Responsibilities:

  • Learn, adhere to, and deliver evidence-based treatment models
  • Establish and maintain effective working relationships with a variety of clients from diverse cultural backgrounds.
  • Establish and maintain effective working relationships with a range of stakeholders, including school, justice, and welfare system personnel, partner agencies, and community members.
  • Conduct strengths-based, detailed, in-home family assessments for clients and their families, as appropriate.
  • Conduct services in schools, homes, CCEB's offices, or in the community.
  • Provide relevant services including facilitated linkages to the multiple services of Catholic Charities of the East Bay and to appropriate community-based resources needed to help clients meet identified goals.
  • Adhere to timely and accurate clinical documentation and data collection requirements
Position Requirements


  • EDUCATION: Master’s Degree from an accredited college or university
  • BBS REGISTRATION: Valid and current registration or licensure as an AMFT, ASW, APCC, LMFT, LCSW or LPCC
  • Knowledge and skills of principles, models, and techniques of intensive clinical case management with a focus on impoverished and violence impacted communities.
  • Cultural relevance in African American, Latino, traumatized populations a must
  • Experience with restorative practices a plus
  • Ability to drive to and from schools and community meetings in personal vehicle and possession of a valid driver’s license, proof of insurance and access to reliable incurred vehicle
  • Fingerprinting and background check with clearance to work with children and youth
