About the Organization Hill Country Mental Health and Intellectual and Developmental Disabilities Centers is a unit of local government and a not-for-profit, 501C provider of Mental Health and Intellectual Developmental Disabilities, Substance Abuse, and Veteran Peer Services throughout the greater Texas Hill Country Region. Hill Country MHDD Centers is designated to be the Mental Health Authority by the Texas Department of State Health Services (DSHS) and the Intellectual and Developmental Disability Authority by the Department of Aging and Disability Services (DADS) for a 19 County Service Area including: Bandera, Blanco, Comal, Edwards, Gillespie, Hays, Kendall, Kerr, Kimble, Kinney, Llano, Mason, Medina, Menard, Real, Schleicher, Sutton, Uvalde and Val Verde Counties. Job Title: Care Coordinator - Children – Mobile Team, Kyle, TX 78640

Essential Duties and Responsibilities:

Hill Country Mental Health and Developmental Disabilities Centers strongly believes that every individual deserves the opportunity to achieve their fullest potential. At the present time, we are looking for a qualified candidate to fill the role of Care Coordinator - Child for the Mobile Clinic. This position is responsible for providing recovery coaching to children receiving mental health services. Utilizing Person-Centered Recovery Planning, the Recovery Coach will:

  • Develop recovery plans
  • Provide skills training
  • Provide linkage and referral services
  • Provide crisis prevention/management services
  • Provide crisis “on call” services on a rotating basis
  • Collaborate with service providers and agencies in order to link individuals with outcome-focused services
  • Consistent in keeping appointments with consumers
  • Contact individuals in accordance with recovery plan
  • Keep accurate and timely documentation using Collaborative Documentation, which supports recovery plan and outcome-based services
  • Perform related duties as assigned by supervisor
  • Works cooperatively with others
  • Be well-organized
  • Maintain compliance with all company policies and procedures

Other Duties:

  • Ability to keep a flexible work schedule
  • Some travel required
  • Excellent verbal and written communication skills, including ability to effectively communicate with consumers and Supervisor
  • Ability to write complex sentences, using normal word order and present and past tenses
  • Ability to perform simple mathematical concepts such as multiplication, division, addition, subtraction, fractions, percentages, rations and proportions to practical situations
  • Ability to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices
Position Requirements

Education and/or Work Experience Requirements:

  • Bachelor’s degree in the Human Service field or related field required
  • One to Two years of experience working with children in the Mental Health Field
  • One year of experience implementing the Adult Needs & Strength Assessment / Child & Adolescent Needs & Strengths Assessment and / or the Texas Resilience and Recovery Model for Adults or Children (preferred)
  • Experience with crisis assessments and intervention (preferred)
  • Experience with treatment planning and clinical documentation (preferred)


  • Texas Driver’s License
  • Current Automobile Insurance

Physical Requirements:

  • Walk, talk, hear, use hands to handle, feel, or operate objects, tools, or controls, and reach with hands and arms
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards
  • Able to push, pull, lift, and/or carry up to 20 pounds
  • Able to talk, listen and speak clearly on telephone

Compensation $16.34 per hour / $34,000 Annual- Eligible for an increase in salary when identified quality performance standards are met!  
On-Call Pay Schedule (If applicable) This position requires flexibility in schedule and some travel. This position is based in the Kyle Mental Health Clinic.
