Description

The Senior Living Community Case Manager (SLCCM) coordinates and oversees the provision of patient care services for residents living in assisted living facilities and senior independent living environments. The SLCCM works collaboratively with senior living staff, patients and their families and provides on-site supervision of the senior living clinical team. The SLCCM is highly focused on providing excellent customer service, education, and visibility in assigned communities. This role requires business development knowledge, clinical care skills, and case management. The SLCCM provides clinical staff oversight and direction for the senior living clinical team according to industry best practices, regulatory and accreditation standards. Demonstrates excellence in clinical outcomes, episode management, re-hospitalization prevention as well as internal and external customer satisfaction according to benchmarks identified by the organization.

Position Requirements

Education: Graduate of an accredited Associate Degree or Baccalaureate School of Nursing.

Licensure/Certifications: Must have current Registered Nurse licensure by the Georgia State Board of Nursing and a current valid CPR certificate. Must possess a current valid Driver’s license.

Previous Experience: Minimum 2 years of home healthcare experience, along with at least 1 year of management experience in a healthcare setting. 1 year of professional experience in senior living environment preferred.

Knowledge/Training: Knowledge of the physical, social and emotional needs of the elderly, disabled and terminally ill. Knowledge of Medicare, Medicaid and other funding source regulations as applied to home health and hospice care services. Demonstrate knowledge and competence in advanced practice nursing skills, including the nursing process and collaborating with an interdisciplinary team. Effectively communicates with persons at various educational and attitudinal levels.

Computer Skills: Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

Transportation: Must have valid automobile insurance and reliable transportation.

Time and Attendance: This is an exempt position and employee must be able to work according to normal office working hours. After hour and weekends may be required to ensure safe patient care delivery is rendered. This position will also require rotation participation in after hours and/or weekend on-call as determined by organization needs.

Continuing Education Requirements: Personnel in this position are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities and to meet minimum State Licensure requirements for continuing education.

Other: The SLCCM will be required to make home/facility visits in order to admit, direct or coach/mentor clinical staff. Maintains yearly ongoing competency requirements.
