About the Organization Hill Country Mental Health and Intellectual and Developmental Disabilities Centers is a unit of local government and a not-for-profit, 501C provider of Mental Health and Intellectual Developmental Disabilities, Substance Abuse, and Veteran Peer Services throughout the greater Texas Hill Country Region. Hill Country MHDD Centers is designated to be the Mental Health Authority by the Texas Department of State Health Services (DSHS) and the Intellectual and Developmental Disability Authority by the Department of Aging and Disability Services (DADS) for a 19 County Service Area including: Bandera, Blanco, Comal, Edwards, Gillespie, Hays, Kendall, Kerr, Kimble, Kinney, Llano, Mason, Medina, Menard, Real, Schleicher, Sutton, Uvalde and Val Verde Counties. Job Title: Case Manager

Position Summary

Hill Country Mental Health and Developmental Disabilities Centers strongly believes that every individual deserves the opportunity to achieve their fullest potential. At the present time, we are looking for a qualified candidate to fill the role of Case Manager.

This position is responsible for identifying individual's needs; development of goals and strategies for the completion of the Implementation Plan. The Case Manager also handles the scheduling, facilitating and documenting of the Implementation Plan and Called Team Meetings. On an annual basis this position handles completing and submitting the annual IPC and any revisions required; completing and submitting the IDRC and ICAP as required; and obtaining prior approval for Adaptive Aids and Minor Home Modifications for individuals in the HCS Program. All services are delivered in compliance with HCS and TXHmL Principles.

Essential Duties and Responsibilities

  • Facilitate and document meetings to identify individual’s needs and steps to address those needs
  • Complete monitoring of service delivery on a quarterly basis
  • Develop and submit documents as required by the Department of Aging and Disability services to maintain individuals’ services and supports
  • Assist individuals with maintaining their benefits
  • Ensure adaptive aids are purchased meeting HCS guidelines
  • Ensure Service Coordinator is notified of individuals needs and changes to service delivery
  • Must be willing to work well with all team members
  • Maintain open communication with individuals, their family members and all team members
  • Participate monthly with on call rotation for residential services
  • Other duties as assigned by Area Director

Position Requirements

Qualifications / Work Experience:

  • High School Diploma / GED
  • Four (4) years of demonstrated experience working with individuals with Intellectual and Developmental Disabilities

Certifications / Licensure:

  • Valid Texas Driver's License
  • Current Automobile Insurance
