About our Agency

Hill Country MHDD Centers, the Local Mental Health Authority is located within the Beautiful Texas Hill Country Region. We are located less than an hour from San Antonio big city attractions and 30 miles from the lovely Wine Country Region in Fredericksburg. HCMHDD Centers provides a host of Behavioral Health Care Services to over 630,000 consumers within a 19 County region covering 22,593 square miles. In addition to our Crisis Stabilization Unit we provide Case Management Counseling, Pharmacological Management, Medication Training and Support, Psychiatric Rehabilitation, Skills Training, Engagement Activities and Supported Employment and Housing. We also provide Children's Mental Health services to youth, ages 3 throughout 17, Substance Abuse Services, Veteran Services and Supports to Individuals with Pervasive Developmental Disorders.

Job Overview:

Under the supervision of the Medical Director and Supervising Physician, the Nurse Practitioner (NP) will perform advanced clinical medical care to adults in a 16-bed Crisis Stabilization Unit and various HCMHDDC out-patient clinics. The work involves assisting, planning and providing comprehensive patient care independently or in collaboration with other members of an interdisciplinary team. The NP will direct medical activities/treatment: monitor trends and activities, making recommendations for improvement.

The NP will work collaboratively with intra-agency teams of psychiatric providers, clinicians, case managers, skills trainers, peers and others. Qualified candidates must be knowledgeable in working with adults with mental health needs.

Essential Duties/Responsibilities:

  • The Psychiatric Nurse Practitioner will provide medication management and consultation with our psychiatric and interdisciplinary team.
  • Possess skills in the care and treatment of patients and in the use of medical diagnostic and treatment tools and equipment
  • Possess knowledge of the principles of development, implementation and documentation of case histories and individualized care of treatment
  • Diagnose, treat and help prevent mental, emotional and behavioral disorders
  • Prescribe, direct and administer psycho therapeutic treatments and medications.
  • Reviews, evaluates and collaborates with other professionals regarding treatment plans, client progress and outcomes
  • Confers on the diagnosis and direction of treatment with client, significant others, agency clinicians and with the client's other treatment providers.
  • Participates in mental health and service utilization audits, peer review and quality assurance activities.
  • Facilitates referrals to appropriate internal and/or external resources where indicated.
  • Reviews, approves and signs Treatment Plans and Assessments for assigned clients.
  • Thoroughly documents diagnosis, treatment and all aspects of such illness(es) in accordance with applicable OARs and industry standards.
  • Submits timely documentation of services provided using Electronic health Records and collaborative documentation.
  • Participates in phone response on-call schedule
  • Manages time and caseload effectively.
  • Candidate must be able to work independently and flexibility, under general supervision

Position Requirements

Education and/or Work Experience Requirements:

  • Advanced Degree in Nursing - Psychiatric APN
  • Licensed as a Psychiatric APN by the Texas State Board of Nursing
  • Experience working in a Community Mental Health setting and ANCC Certification as a Psychiatric - Mental Health CP - Preferred

License/Certificates:

  • Valid Texas Driver's License
  • Current Automobile Insurance
