About the Organization Hill Country Mental Health and Intellectual and Developmental Disabilities Centers is a unit of local government and a not-for-profit, 501C provider of Mental Health and Intellectual Developmental Disabilities, Substance Abuse, and Veteran Peer Services throughout the greater Texas Hill Country Region. Job Title: Care Coordinator - Child

Looking To Make A Difference? Hill Country Mental Health and Developmental Disabilities Centers has an immediate opening for a qualified candidate to fill the Mental Health Care Coordinator – Child position!

Position Summary: The Mental Health Care Coordinator – Child is responsible for providing services and supports to children with mental illness in order to assist the individual in reaching their vision of recovery. The Care Coordinator will assess the individual’s strength and needs and in collaboration with the individual develop a Person-Centered Recovery Plan. The Care Coordinator will provide skills training; linkage and referral; crisis prevention/management, Supported Housing and Supported Employment services. The Care Coordinator is responsible for scheduling and carrying out individual and group rehabilitation services and case management. This position will be required to participate in a crisis on-call rotation and be available for face-face assessment within 1 hour of notification on assigned days. Office hours are 8:00 am – 5:00 pm. (Includes occasional weekends, holidays and evening work) to accommodate individuals that cannot meet between traditional office hours. The Care Coordinator must follow agency policy and procedures and is expected to meet performance standards.

Essential Duties and Responsibilities:

  • Ensures that all individuals on caseload have a current Uniform Assessment (at least 95% of all CANS / ANSA’s are completed on time)
  • Achieves an average score of 80, with no one score below 75 on the “Person Centered Recovery Plan” Quality Management audits
  • Schedules and delivers at least 240 service hours per quarter; average of 80 per month
  • Responsible for client no-show rate of 25% and staff kept appointment rates of at least 90%
  • Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service
  • Achieves 65.6% on Adults and 65% on Children’s engagement target
  • Provides average hours as required by Level of Care served
  • Achieves 3% of Supported Housing and Supported Employment targets
  • Provides at least 75% Person Centered Time each month
  • Will deliver services using the recommended Evidenced Based Practice recommended by HHSC

Position Requirements

Minimum Qualifications / Work Experience:

  • Bachelor’s Degree from an accredited college or university in the social, behavioral or human services field (Please submit copy of college transcripts and resume with application)

Preferred Qualifications:

  • One to two years of experience working with Children in the Mental Health Field
  • One year of experience implementing the Adult Needs & Strength Assessment and the Resilience and Recovery Model
  • Bilingual (English/Spanish) Speaking
  • One year of experience implementing Adult Needs & Strength Assessment / Child and Adolescent Need and Strengths Assessment and / or the Texas Resilience and Recovery Model for Adults or Children

Certificates/ Licenses:

  • Valid Texas Driver’s License
  • Current Automobile Insurance

Compensation $16.34 per hour - $34,000 annually This position may be eligible for an increase in salary when identified quality performance standards are met!
