Position: Youth Facilitator
Site: Holland Bloorview Kids Rehabilitation Hospital
Department: Transition Strategy
Hours: Days, 15 hours per week - 37.5 hours per week
Status: temporary Part-time

JOB SUMMARY

We are seeking a young adult who has lived experience with a physical disability and has been a patient in both paediatric and adult healthcare settings. There is currently 1 position open. The role requires flexibility in schedule as some shifts will be during weekdays, evenings and weekends.

The Youth Facilitator will meet with clients individually, in small groups and workshop settings. The Youth Facilitator must be able to work independently and as part of the healthcare team. The Youth Facilitator will: initiate and facilitate conversations with clients which may include sharing personal experiences; collaborate with team members in an inter-professional environment providing client-centred care; communicate effectively with clients, families, and colleagues; participate in program planning, development, and evaluation; participate in professional and organizational initiatives supporting quality management and research. Please be sure to give some indication that you fit the criteria for this consumer role. Preference will be given to candidates who have received paediatric rehabilitation services. QUALIFICATIONS
This is a consumer role - must have experience as a patient in paediatric and adult healthcare settings. We are recruiting young adults with physical disabilities for these roles.

  • Post-Secondary Diploma or Degree (or actively working towards with minimum 1 year completed)
  • Knowledge of healthcare systems and community resources commonly used by youth with specialized healthcare needs preferred
  • Must be willing to share personal experiences of navigating adult life
  • Personal experience with independent living, using transportation, volunteering or working and relationships is preferred
  • Experience facilitating groups
  • Comfortable sharing opinions as part of a team
  • Able to observe and run workshops as well as provide insight into development
  • Enthusiastic about working with youth with specialized health care needs
  • Fun and Creative
  • Good organizational and written communication skills
  • Able to use Microsoft Office (Word, PowerPoint, Excel)
  • Must be independently mobile
  • Good attendance is required
  • Holland Bloorview Youth Mentor training an asset

Notes:

The LIFEspan position is not open to current clients of Holland Bloorview Kids or Toronto Rehab's LIFEspan service that have received services from Holland Bloorview in the last two years, except for Employment support program clients
