What are we looking for in a SUDP Care Navigator?

SUDP Care Navigators work within our Hub & Spoke team to provide substance use treatment and case management to clients. The Hub & Spoke model is a network of community providers that work together to provide integrated care for individuals receiving medicated assisted treatment for opioid use disorders. This integrated approach assists clients with multiple facets of their recovery, regardless of which service provider forms the initial connection. The SUDP Care Navigators provide case management for clients to assist with accessing necessary community resources, complete substance use assessments, and utilize motivational interviewing techniques to engage clients in treatment and encourage their recovery from addiction.

SUDP Care Navigator duties may include:

  • completing substance use assessments
  • assessing clients for safety and risk to self or others, and taking appropriate crisis intervention measures when indicated
  • monitoring clients’ progress with symptoms and response to medications
  • advocating for clients to attain services and benefits to further recovery goals
  • collaborating with the client to develop a comprehensive treatment plan that includes each of the services being utilized
  • thoroughly completing all documentation in a timely, client-centered manner
 
Position Requirements

Education & Experience: Associate degree or certificate in Substance Use Disorder Treatment or Addiction Studies

Professional Credentials: SUDP or SUDPT credential in Washington State

Other Requirements: Driver's license, vehicle liability insurance
