We are seeking mission focused professionals to serve as a full-time Case Coordinator/Therapist responsible for providing in-home support services for the Decatur, Alabama Division's foster parents and children.

As the Case Coordinator/Clinical Therapist, you will;

  • Participates in the intake process for children/families assigned to your caseload.
  • Work independently while having a flexible schedule and providing direct services to include basic living skills, psychoeducational, individual/family counseling, and crisis intervention services to families and children assigned to caseload as deemed necessary by the child’s treatment and in conjunction with the ISP.
  • Train and coach Biological/Foster parents to work successfully with therapeutic and or biological children while in a home setting.
  • Provides crisis intervention while participating in our rotating on-call schedule.
  • Provide weekly face to face in-home contact with child/family according to ISP/treatment plan to include evenings.
  • Assist the client/family and DHR with identifying permanency options to be outlined on the client’s treatment plan and the ISP.
  • Make contact with client’s school, DHR worker, and attend ISPs and IEP meetings as scheduled and or needed.
  • Serves as liaison between biological family, foster family, school personnel, DHR, courts and other community resources by encouraging a team approach for treatment, service delivery, and interventions.
  • Connects assigned families to community services and resources they need.

Compliance/Reporting:

  • Generally, hold a caseload that consists of 6-12 clients with whom you visit weekly.
  • Meets timeliness documentation criteria by recording all Mental Health services in our Electronic Record system within 24 hours of the event’s occurrence.
  • Meets production standards by providing and billing direct services to children/families on caseload at a level consistent with established criteria by the state and or division of employment
  • Follow monthly and quarterly requirements as defined annually by STARS standards.
  • Review all youth files as part of the quality improvement process.
  • Maintain contact with referral agency caseworker as outlined by employed state and or division
  • Overall hours worked outside of the office are estimated to be around 60%, which includes traveling to and from visits and time spent in the home conducting a home visit.
  • Travel to family’s homes in our Northeast and Northwest Alabama- mileage reimbursement included.
 
Position Requirements

What you bring to this role;

  • Master’s degree in social work, human services, or related field
  • 1+ years of experience working with families and children, child welfare, and in-home therapies
  • Passion for serving and improving the lives of traumatized children/youth in foster care
  • Flexible schedule must work evenings, and weekends as needed to serve children and families
  • Good written and verbal communication skill
  • Good with technology
  • Strong time management skills
  • Work as part of a team
