Our Ryan White Part B Nurse will assist the Infectious Diseases treatment team by triaging walk-in patients and phone calls, providing health education and linkage to internal and external resources, evaluating and managing the care and education of patients and overseeing the day-to-day activities of patient care. This would include assessing patients, administering injections, developing comprehensive, indiviaulized care plans for the RWPB eligible patients, read and document test results and assist with medication management. This role also serves in a case management capacity including assessment of client needs, coordinating delivery of services, evaluation of care plan, and coordination with other staff (i.e. social services, physicians, Physician Assistant, and pharmacist) and the medical community at large. You’ll assist the treatment team where needed and update and maintain health maintenance forms. You’ll have the opportunity to assist wherever needed in our fast-paced clinic and by doing so, expand your experience.
