Share Email Opening Apply Now Title Community Support Specialist - Bachelor's Degree in Human Services  Description This position is responsible for providing a wide variety of community based support services to individuals with a Serious Mental Illness (SMI) designed to reduce and/or prevent emergency department visits, mental health unit admissions or readmissions and placement in a more restrictive environment. Supports and encourages the use of daily living skills training including but not limited to development of medication management strategies, shopping, learning the transportation system, symptom management skills, locating housing, coping strategies, transportation to selected appointments, etc.Provides a wide variety of care coordination or case management activities including but not limited to information and referral, education, setting up appointments, setting up transportation, working with others involved in the client's life, responding to crises, etc. Documentation of all services provided in an Electronic Health Record according to standards, insurance and/or managed care organization requirements, state standards, etc.   Position Requirements Qualifications Bachelor's Degree in a Human Service related field; exemption from the state can be requested if no degree based on experience, training, and education. 1-3 year's preferred experience in the mental health field with A passion to serve others and a desire to be a positive contributor to Waubonsie Mental Health and the communities it serves.   Full-Time/Part-Time Full-Time   Shift Days   Position Community Support Specialist   Req Number CAS-19-00006   Location Clarinda   About the Organization ABOUT US Waubonsie Mental Health Center strives to empower people to achieve their hopes, dreams, and quality of life goals by delivering quality, integrated, trauma-informed behavioral health care.
