About the Organization Hill Country Mental Health and Intellectual and Developmental Disabilities Centers is a unit of local government and a not-for-profit, 501C provider of Mental Health and Intellectual Developmental Disabilities, Substance Abuse, and Veteran Peer Services throughout the greater Texas Hill Country Region. Hill Country MHDD Centers is designated to be the Mental Health Authority by the Texas Department of State Health Services (DSHS) and the Intellectual and Developmental Disability Authority by the Department of Aging and Disability Services (DADS) for a 19 County Service Area including: Bandera, Blanco, Comal, Edwards, Gillespie, Hays, Kendall, Kerr, Kimble, Kinney, Llano, Mason, Medina, Menard, Real, Schleicher, Sutton, Uvalde and Val Verde Counties. Job Title: Care Coordinator - Adult and Child (QMHP)

Essential Duties and Responsibilities

This exciting and innovative team is seeking a Care Coordinator at the Uvalde County Mental Health Center to work with both adult and children populations. Utilizing Person-Centered Recovery Planning the Care Coordinator will develop recovery plans; will document services provided collaboratively with individuals in services, will provide skills training, linkage and referral, crisis prevention/management, crisis on-call services on a rotating basis and will collaborate with service providers and agencies in order to link individuals served with outcome-focused services.

This position requires frequent and proficient use of computer skills, minimal flexibility in schedule and some travel. On-call crisis coverage will be required on a rotating basis which is compensated.

**After meeting the performance standards two consecutive quarters, the Care Coordinator is eligible for a pay increase.

Position Requirements

Minimum Qualifications:

  • Bachelor’s Degree from an accredited college or university with at least 30 hours of course credit work in the following areas of study: Counseling, Education Psychology, Gerontology, Human Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Work, Sociology
  • Position requires candidate to meet QMHP Credentialing Guidelines

Preferred Qualifications:

  • Bilingual: English/Spanish Speaking
  • One to two years of experience working with children in the Mental Health field.
  • One year of experience implementing the Adult Needs & Strength Assessment (ANSA) / Child & Adolescent Needs & Strengths Assessment (CANS) and / or the Texas Resilience and Recovery (TRR) Model for Adults or Children
  • Computer proficient - intermediate to advanced with Microsoft Office Products

Compensation $16.34 per hour / $34,000 - After meeting the performance standards two consecutive quarters, the position is eligible for a pay increase.
