SAFY of Birmingham, Alabama, is seeking a full-time IL Case Coordinator who will be responsible for the case management, coaching, and skill-building services of a caseload of young adults living in community-based housing.

As the Case Coordinator, you will;

  • Assist young adults assigned to caseload with working toward achieving goals of employment, pursuing college and or vocational training within 90 days of service
  • Completes client intakes including determining the diagnosis and preparing treatment plans
  • Provides independent living skills (individual and group) according to the treatment plan and family support (as identified) and crisis services to the client
  • Following policy provides client crisis intervention & on-call services designed by the division or state
  • Meets with the client in ILP apartment based on needs, including evening and weekend visits as deemed necessary by safety, ILP and treatment plan tea
  • Provides face-to-face contact/services according to the program guidelines
  • Provides aftercare services as identified in program guidelines
  • Backs up co-workers and performs other duties as assigned
  • Attends ISP, IEP, (court if subpoenaed) and other treatment meetings
  • Updates DHR on client progress toward independence
  • Investigates and identifies extended family and other possible resources so to establish a support system for the client as defined in program guidelines
  • Encourages a smooth transition from out of home care to permanence for each assigned client using Home to Stay model, birth or resource families and DHR
  • Uses a range of interventions and modalities to deliver service
  • Engages with DHR to encourage/secure new referrals as needed
  • Caseloads size- will vary from state to state depending on the service standards.
  • Compliance/Reporting:
  • Timely document outcome of home visits, support sessions, interaction with parties involved in the youth’s care and incidents as required by policy, regulations, and contract requirements. Prepare and submit timely all the necessary company, RFP, program, referral agency and regulatory reports, progress notes, and billing tickets. Compile profile using pre and post assessment tools for outcome measures. Report census information to the DOM/finance department for billing as indicated.
  • Maintain contact with referral agency caseworker using discharge reports, Diagnostic Assessment forms, telephone calls, and incident reports, also, regularly scheduled and required written reports. Review all youth files as part of the quality improvement process.
  • Follow monthly and quarterly requirements as defined annually by STARS standards. Meet utilization standards for Mental Health services as described in the Utilization Policy
  • Inform youth of company policies, procedures, rights, and responsibilities. Enforce policies by writing corrective action plans, incident reports, and conducting rules investigations. This task may not be carried out by Case Coordinators in all states.
 
Position Requirements

What you bring to this role;

  • Bachelor’s degree in the field of social work, counseling, psychology, sociology, child & family studies, or other approved related field of human services
  • Licensure: Preferred
  • Two years of relevant experience with above-listed degrees, experience in child welfare preferred, OR relevant experience in working with children in general.
  • Five years of relevant experience required with the related field of human service degrees
