Description

The Supportive Services for Veteran Families (SSVF) program offers Homelessness Prevention & Stabilization Services to Veterans and their families. These supportive services include outreach, case management, help in obtaining VA benefits, temporary financial assistance for rent and utilities and assistance in obtaining and coordinating other public benefits.

Job Title:Case Manager

FT/Exempt Position/Full Benefits Package; Health Benefits available the first of the month following date of hire.

Schedule: 9-6 (Monday - Friday); Evenings and Weekends as necessary ; (50% out of office)

Employment Duration: 1 year

Essential Job Responsibilities:

  • Provide encouragement, direction, support and motivational tools to Veterans experiencing homelessness and/or facing eviction
  • Completes service plans with Veterans including housing, employment, benefits and other related needs and resources
  • Coordinates and monitors services including comprehensive tracking of Veteran activites in relation to service plan and housing inspections/home visits
  • Advocates for and facilitates Veteran's access to community resources such as housing assistance, utility assistance and relevant community services and benefits
  • Provides referrals to community rescources as need
  • Network at monthly Veteran community groups
  • Network with verteran service organizations
  • Collaborate with Lead Case Manager
  • Average case load of 20-30 Veterans per month
 
Position Requirements
  • Bachelor’s Degree in Social Services or related field required
  • 2 years of case management experience
  • 2 years in a homeless related field required
  • Experience with Veterans and their families a strong plus
  • Bilingual Preferred
  • Must have own transportation; travel required/drivers license with clean driving record
  • Strong communication and computer skills
  • Ability to work in a remote office, self motivating and sufficient
  • Must have strong ethics and professionalism
  • Ability to work in a fast paced/team environment
  • Must meet physical requirements of the job
