REACH Hospital Liaison

Position Summary:

The REACH Hospital Liaison provides support to REACH clients who are hospitalized at state or local hospitals within the DBHDS DD Western Region. This position serves as the primary contact for discharge planning and facilitates admission to the REACH Crisis Therapeutic Home or REACH Mobile Crisis Supports if appropriate. This position will also serve as the primary Coordinator for REACH services, which may include the completion of a comprehensive intake assessment, until the case is fully opened and assigned for on-going REACH services.

REACH is a crisis prevention, intervention and stabilization system to support individuals with intellectual and/or developmental disabilities, as well as a mental health condition or challenging behavior that is negatively affecting their quality of life. REACH provides consultation, prevention, development of crisis education & prevention plans, community based mobile support and crisis therapeutic home services.

Essential Job Functions:

  • Serves as primary point of contact between state and local hospitals, REACH, families, and/or other community providers for individuals who are eligible for REACH Services.
  • Completes referrals to any REACH CTH in Virginia when appropriate.
  • Completes referrals to REACH Mobile Crisis Supports when appropriate.
  • Participates in routine client meetings for discharge planning purposes.
  • Provides Crisis Education and Prevention Plan (when applicable) to state and local hospital staff and provide training and technical assistance as required.
  • Develops and/or revises Crisis Education and Prevention plans for individuals when applicable.
  • Speaks to provider, family, hospital and/or community groups representing REACH services.
  • Participates in required meetings with Team Members, REACH Director, CTH staff and other applicable parties.
  • Identifies and coordinates training and technical assistance needs for providers, natural supports and other components of the system of care.
  • Travels at least 70% of the time to local and state hospitals
  • Provides 24/7/365 response to crisis situations according to on-call rotation schedule.
  • Makes accurate assessments of prospective admissions to REACH CTH and/or Mobile Crisis Supports
  • Attends REACH related meetings, conference calls and events.
  • Collects and enters required data and produces documentation on service delivery and overall utilization of REACH services within established timeframes and according to set protocols.

Position Requirements

Minimum Qualifications:

  • Minimum of a bachelor’s degree in human services or related field from a USDOE or CHEA accredited institution and no less than one year of fulltime relevant experience
  • Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP)
  • Graduation from an accredited college/university with a Master’s Degree in Counseling, Social Work, Psychology, or closely related field preferred.
  • Licensed or Licensed-eligible strongly preferred.
  • A minimum of 3 years’ experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs preferred.
  • Experience with crisis prevention and intervention services preferred.
  • Knowledge of Virginia Hospital and Community Based services.
  • Has strong verbal and written communication skills; has the ability to speak professionally in team settings and to represent REACH services
  • Maintains valid, unrestricted, state-appropriate driver’s license and insurance for personal vehicles utilized for work.
  • Completes within specified time period the trainings as required by agency and service model.
