Pharmacy Intern

A Pharmacy Intern is responsible for performing administrative duties, tasks, and assignments supporting either SinfoníaRx administrative operations or SinfoníaRx Medication Therapy Management (MTM) programs and services. The assigned responsibilities primarily focus on providing medication management therapy services to patients with all types of medication management needs, such as: asthma, diabetes, COPD, and CHF.

A Pharmacy Intern works with intermittent supervision, possessing limited and defined decision-making authority for resolution of normal and routine work problems within the scope of authority and responsibility for the position. Processing errors involving departures from standard policies, interpretations, or procedures are reviewed with the supervisor or a licensed pharmacist for direction and resolution.

ESSENTIAL JOB FUNCTIONS:

„h Under the direct supervision of a pharmacist, provides medication management services to patients and clients of SinfoníaRx, such as:

o Performs community health screenings with patients;

o Completes comprehensive medication reviews live and over the phone;

o In support of the Medication Management Center (MMC):

„X Makes outbound "cold calls" offering medication management services to patients;

„X Provides counseling via a structured interview format as required by the program sponsor and clients. The structure of counseling sessions may vary depending on program sponsor.

„X Makes therapeutic recommendations to optimize medication regimens including, but not limited to: generic substitutions, duplicate therapy, and therapeutic interchanges;

„X Contacts the patient's prescriber(s) to authorize therapeutic interchanges, reviews potential duplicate therapies, and/or consults with the prescriber as appropriate and conveys any patient therapeutic change authorizations from prescribers as well as any other recommendations for therapy modifications to the patient's dispensing pharmacy;

„X Documents all services provided and recommendations made including declination of service by patients;

„X Maintains performance standards including, but not limited to: call volume consistent with other Medication Management Center pharmacists.

„h Provides day-to-day support for SinfoníaRx staff and/or its operations, including general administrative, research, and reporting tasks and assignments.

„h Provides research and informational support for existing and future program or service development.

„h Provides administrative support for Sinfonía Health Care related community outreach programs.

„h Assists and supports quality assurance and quality improvement projects, as assigned.

EDUCATION:

  • Must be enrolled in, or have completed a, formal course of study to become a pharmacist from an approved and accredited college or university.

EXPERIENCE:

  • Preferred: prior experience working in a MTM call center environment and/or with a geriatric population in a healthcare environment.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Possess and maintain a current Pharmacy Intern License from the respective and applicable State Board of Pharmacy.

OTHER SKILLS and ABILITIES:

  • Intermediate skills and understanding of computer software, healthcare systems and programs, and Microsoft Office: specifically MS Excel.
  • Ability to communicate at effectively and professionally through written, verbal, and interpersonal skills as applied when interacting with employees, clients, or agency representatives; successfully conveying and exchanging information in a positive manner.
