Community Resource Trainer -Innovations (Job Coach)

***Monday -Friday, 8am-12pm (20 hours per week, minimum)***

Position Summary:

The Community Resource Trainer (CRT) performs as an integral part of the management team working with adults with disabilities. This position can be full time, part time or periodic with primary duties to include providing direct care service and supports to individuals with disabilities in community based employment. The CRT will provide services and supports based on the need of the individual as designated in their service plan and funded by Innovations and/or LTVS. The CRT will act as the primary contact with the employer and will complete monthly satisfaction surveys with the employer and individual.

Essential Job Functions:

  • Provides follow along services for individuals that have completed intensive training phase of job coaching.
  • Develops and implements quarterly reviews on Individual Service Plans for each individual assigned.
  • Provides a minimum of two job site visits to employers of individuals each month.
  • Conducts employer and individual satisfaction surveys every month for all employed individuals assigned.
  • Maintains case records as required by licensing and funding standards for NC MR/DD/SAS, Medicaid, etc. Documentation must be entered into the electronic medical record system within 24 hours of service delivery.
  • Provides training and counseling interventions as needed to assist individuals to maintain community based employment.
  • Facilitates social interaction with coworkers of individual being supported and provides reinforcement as learning is demonstrated.
  • Assists with the personal care needs of the individuals being supported such as toileting, dressing, eating, etc. as needed.
  • Coordinates communication with parents/guardians, care coordinator and other agencies serving the individuals being supported.
  • Maintains good work habits regarding attendance, punctuality, appearance and effective working relationships with co-workers, employers and individuals being supported.
  • Provides daily training interaction with individuals being supported according to their service plan and assigned work tasks.
  • Responds to safety needs of the individuals being supported (i.e. choking, positioning, etc.)
  • Keeps the Team Leader informed on all issues of importance.
  • Attend appropriate work related workshops, in-service trainings, and conferences.
  • Complete required training within prescribed timeframes.
  • Maintains regular attendance and punctuality, manages use of PTO time, and follows all policies and procedures regarding time off.
  • Follows all ESUCP Policies and Procedures, including Universal Precautions practices.
  • Demonstrates Easter Seals valued behaviors of Accountability, Commitment and Caring as appropriate to position.
  • Performs related duties as requested.

 
Position Requirements

Minimum Qualifications:

  • High School diploma or equivalent appropriately accredited by the appropriate state department of education
  • A minimum of two (2) years of experience working with adults with disabilities is preferred.
  • Knowledge of the labor market and community services agencies as well as skills in writing task analysis, systemic job training, counseling, advocacy and supported employment are preferred.
  • A valid North Carolina driver’s license, good driving record and be insurable is required.
  • Must have reliable transportation that will allow movement throughout the program delivery area.
