Our Child Placement Services program has a new opportunity for a Child Placement Therapist / Coordinator to join their team in Jacksonville, NC. This unique Full-Time role will provide combined services of 50% Therapy and 50% Coordination for active caseload. Flexible hours with rotating 1-monthly On-Call coverage required.

Child Placement Therapist:

The Child Placement Therapist provides direct therapeutic interventions as part of a multi-disciplinary team intended to stabilize the living arrangement and promote reunification for identified youth through the age of 20. These services are provided in multiple locations, including therapeutic foster homes, family-household and community of the person served.

  • Provide weekly individual and family therapy, including the reunification family and treatment foster family, in a community-based or home-based setting.
  • Provide crisis intervention services and psychoeducational support regarding wellness, recovery, and substance abuse.
  • Assists in transition in-and-out of services, working in cooperation with other providers/agencies and continuation of individual therapy as appropriate after discharge from foster care.
  • Ensure all children receive Comprehensive Clinical Assessments upon placement and annually thereafter.
  • Attend Child and Family Team meetings for individuals served.
  • Participates in the development and implementation of the person-centered plan.
  • Provides supportive counseling and psychoeducational support to the child and family to improve stability and coping mechanisms in accordance with evidence-based interventions.
  • Works with caregivers in the implementation of home-based behavioral supports.
  • Helps monitor presenting behavioral and/or addiction symptoms.
  • Provides psycho-educational support regarding wellness, recovery, and substance abuse.
  • Assists in transition in and out of services, working in cooperation with other providers/agencies and continuation of individual therapy as appropriate after discharge from foster care.
  • Provides parent skills training to encourage family skill base around crisis response and interdependence with community resources.
  • Attend weekly staff meetings and provides supervision to team members working with youth receiving therapy. Submit weekly supervision reports to direct supervisor.
  • Documents services and service provision according to ESUCP, state and federal guidelines.

Child Placement Coordinator:

The Child Placement Coordinator functions as the coordinator of placement services for children receiving treatment foster care. Responsibilities include intake, casework, service planning, and planning/coordinating the services and resources affecting children and their families.

  • Advocates for personal outcomes for children and families, adhering to best practices.
  • Coordinates admission and discharge planning with respective contracted agencies including completion of required documents prior to placement and conducts an initial visit with the child within 72 hours.
  • Assists the Licensing Coordinator with recruitment and provision of training for prospective foster parents.
  • Supports children, families, foster parents and contract agencies by responding to calls promptly and making daily / weekly visits.
  • Communicates with collateral agency representatives regarding child/family status including medication changes, residential situation, staff changes, school performance, incidents, etc. (within 24 hours)
  • Assures adherence to Client Rights and Confidentiality Rules including HIPAA regulations.
  • Prepares and maintains service records on children and families in accordance with NC DSS Administrative Rule 10A NC 70 and DMA documentation standards.
  • Ensures all required electronic data is complete, correct, and entered into Client Care Web within required timelines for Intensive Alternative Family Treatment level children.
  • Transports individuals being served, as needed.
 
Position Requirements

Minimum Qualifications:

  • Master's degree in a human service field from a college or university accredited by an entity approved by USDOE or CHEA
  • Must hold an active, unrestricted license or provisional license as an LCSW, LPC, LMFT, or LCAS
  • Credentialed with MCO’s preferred.
  • Have documented previous work experience with the population to be served.
  • Maintain valid, unrestricted driver’s license and valid insurance for personal vehicles utilized for work.
