We provide support services and advocacy to 400 adults and children and their families in Harford and Cecil Counties. The Arc supports people with intellectual and developmental disabilities to create lives that mirror yours and mine: students attending neighborhood schools, adults going to work and living in their own homes, and individuals enjoying the social and recreational opportunities in their communities. We are a private, non-profit local chapter of The Arc of Maryland and The Arc of The United States, the largest volunteer organization in the world devoted exclusively to improving the quality of life for all adults and children with developmental disabilities.

Description

Position Summary:

The Case Management Supervisor is responsible for the oversight of Case Management, supervision of the Case Managers and ensuring supported people are working towards their individual goals. They assist in identifying community resources, provide support and intervention where needed and act as a liaison with family members. The Case Management Supervisor may have a caseload at times and provide additional support to Case Manager's caseloads as needed. In addition, the Case Manager Supervisor will be an advocate for the individuals and ensuring all necessary services are provided.

In a manner consistent with supporting The Arc NCR Mission Statement the Case Manager Supervisor shall do the following:

Quality Supports

  • Ensures that all Person-Centered Plans are completed according to The Arc NCR policy and that they are fully implemented by Case Managers and tracked.
  • Works in conjunction with other organizational departments to ensure the needs of individuals served are met utilizing all resources available.
  • Facilitates team meetings and family conferences to problem solve and develop plans of action as needed.
  • Coordinates all services the person is receiving and advocates internally and externally for the supports needed.
  • Develops an active Circle of Support around the individual and maintains communication and positive working relationships with the people who are involved in the lives of people supported, including people associated and not associated with the agency, professionals, friends and family members.
  • Review and approve data for supported individuals including behavior support plans, incident reports and other tracking metrics
  • Attend and support individuals in planning meetings to establish goals and outcomes of their choosing
  • Collaborate with all appropriate team members to assess potential admissions into the programs.
  • Provides advocacy for individuals served based on their personal choice.
  • Prepare and maintain records and case files, including documentation such as Individuals' personal and eligibility information, services provided, narratives of individual contacts, and relevant correspondence.
  • Monitor and record individuals' progress to ensure that goals and objectives are met.
  • Confer with individuals to discuss their options and goals so that organizational programs and plans for accessing needed services can be developed.
  • Maintain close contact with individuals during job training and placements to resolve problems and evaluate placement adequacy.
  • Analyze information from interviews, educational and medical records, consultation with other professionals, and diagnostic evaluations to assess individuals' abilities, needs, and eligibility for services.
  • Locate barriers to client employment, such as inaccessible work sites, inflexible schedules, and transportation problems, and work with individuals to develop strategies for overcoming these barriers.
  • Provides flexibility as needed.
  • Reviews and approves Person Centered Plans and Periodic Reviews
  • Has an understanding of the Waivers, services The Arc NCR is licensed to provide and impact of the services.
  • Ensure people supported are being provided services agreed upon.
  • Collaborate with individuals' families to implement IP plans such as behavioral, residential, social, and employment goals.
  • Participate in job development and placement programs, contacting prospective employers, placing individuals in jobs, and evaluating the success of placements.
  • Arrange for on-site job coaching or assistive devices, such as specially equipped wheelchairs, to help individuals adapt to work or school environments.
  • Respond to QE requirements by the deadline given
  • Ensure staff are documenting all incidents in ICM within 24 hours.
  • Review and oversight that nursing 45-day reviews outstanding items are addressed.
  • Oversight of Modified Service Funding Plans, Service Funding Plans and any changes to a person's services.
  • Ensures Case Managers are training the appropriate team members on a person's Person Centered Plan.

Quality Team Members

  • Develop and maintain relationships with community referral sources, such as schools and community groups.
  • Manage caseloads
  • Lead and guide monthly meetings to ensure staff are educated and knowledgeable on organizational policies and produces for consistent communication
  • Conduct and implement performance reviews and corrective actions to ensure accountable and professional team members
  • Educate and train newly hired staff and individuals served on organizational policies and procedures for the purpose of talent management
  • Schedule and track team member required organizational training and education needs
  • Supports Case Manager's caseloads in the absence of a Case Manager.
  • Attend and participate in monthly career fairs in order fill vacancies within the case management team.
  • Interview applicants who apply via website or outside of the career fair.
  • Participate in mediation meetings with Case Managers and HR Manager.
  • Complete termination meetings/calls with HR Manager as necessary.
  • Review staff training deadlines with staff to ensure they avoid suspension.
  • Review Safety Alerts from HR and ensure staff sign and complete needed trainings.
  • Review GPS Alerts from Transportation and schedule staff to come in for corrective action.
  • Meet new staff during orientation week during lunch on Monday.
  • Assist in the training and onboarding of new Employment team members as necessary.
  • Completes, maintains, and submits required reports and documentation in a timely manner.
  • Ensure compliance with all regulatory and accrediting bodies.
  • Takes responsibility for safety hazards in all areas including, but not limited to, administrative buildings, and vehicles.

Financial Excellence

  • Completes, maintains and submits required reports and documentation in a timely manner.
  • Approve and submit attendance records for team members in an efficient and timely manner
  • Reconcile and approve mileage reimbursement for vehicle management and fiscal responsibility
  • Works closely with the Admissions Specialist to ensure services align with funding.
  • Work with Case Managers to ensure their schedules are followed and that adequate supports are maintained, while staying in OT budgeted hours.
