Salary: $13 per hour

What You Will Be Doing

This position will provide direct services in Medina County, Ohio – including Day Habilitation and Non-Medical Transportation Services – for adults with developmental disabilities. We pride ourselves on providing a safe and well-structured environment while embracing a person-centered and community integrated philosophy. Our program is community based – so expect outings with the people we support.

Primary Job Responsibilities

  • As assigned, assist in the general referral, admission and transition of persons served into and within the organization via participation in staff meetings
  • Attend ISP meetings as assigned, and implement all sections of the ISP
  • Check and record medical needs of consumers – including nursing delegation and any self-medication programs
  • Provide transportation to consumers, as needed or assigned
  • Assure that proper procedures are followed in regard to consumers and their inherent rights
  • Produce daily progress notes for each consumer and communicate important information by all appropriate means
  • Assist in the periodic review and update of consumer files
  • Assist as needed with ambulation and/or transfer of consumers with mobility impairments
  • Attend staff meetings, and other meetings, as assigned
  • Maintain safe and healthy conditions in the building and vehicles and report maintenance needs to the Program Manager promptly
  • As assigned by the Program Manager, complete all required documentation related to funding, employment or general operations including but not limited to the following: Time reporting, Service delivery, Skill development, Unusual and major unusual incident reporting, and Other incident reports as needed
  • Assist in the timely submission of all records, files, financial records, etc. pertaining to specified individuals
  • Perform all other duties as assigned

This job operates in a classroom type environment.

Position Requirements

Required Education and Experience

  • High School Diploma or equivalent.
  • Two years’ experience working with people with disabilities.
  • Valid Driver’s License and Insurable Driving Record.

Preferred Education and Experience

  • Five years’ experience working with people with intellectual and developmental disabilities.

Additional Eligibility Qualifications

  • Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with consumers and other staff.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
