Easterseals UCP is a nonprofit service provider with over 20 different service lines. We manage residential group home program, summer camps for children with IDD and autism, find employment for people with mental and psychical disabilities, afterschool programs, adoption and foster services, crisis services, among many others. One of our service lines is: VA REACH

REACH is a program to support youth and adults with an intellectual disability and/or development disability, as well as a mental health condition or challenging behavior that is negatively affecting their quality of life. REACH stands for Regional, Education, Crisis Services, Habilitation. REACH emphasizes the prevention of crisis before they occur. This is done through early identification of individuals, development of crisis response plans, training, and technical assistance. The underlying philosophy of REACH is that people's lives go better when everyone involved in supporting the individual works as a team to develop ideas and strategies that will work best of that person.

HOURS: Monday - Friday 9:00 am - 5:00pm

Essential Job Functions:

Support the health and medication needs of guests admitted to the Resource Center.

Coordinate medication orders and ensure accuracy for guests being admitted to the Resource Center

Develop Medication Administration Records

Check medications and verify orders at admission

Obtain vital signs at admission

Monitor health issues and conditions for guests during Resource Center stay

Observe staff for accuracy in medication administration after they have received medication administration training by the RN

Observation of staff’s ability to use medical equipment and manage health conditions (e. g., CPAP, Epi-Pen, Nebulizer, seizures) after staff have received training by the RN

Administration of medications and treatments as needed

Work in collaboration with RN for questions or assessment as needed

Some travel may be required

Other Job Functions:

Teach CPR/FA/AED for all staff

Assisting with blood glucose level checks and insulin administration as needed

 
Position Requirements

Minimum Qualifications:

  • Graduation from an accredited nursing program and licensed as a Practical Nurse in Virginia (LPN)
  • Maintain certification in CPR/FA/AED
  • Maintain nursing license and obtain required continuing education
  • Experience in intellectual/developmental disabilities preferred
  • Knowledge of the MH and I/DD systems of care preferred
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, web-based data entry, etc.
  • Must demonstrate a capacity to effectively communicate verbally and in written form.

Essential physical requirements:

  • Must be physically able to lift a minimum of 50 pounds, bend and assist individuals with mobility needs.
  • Must be able to hear within normal ranges
  • Must be able to talk in order to articulate crisis prevention, intervention and de-escalation supports.
  • Must be able to see with or without accommodation to observe individuals.
