Easterseals UCP is a nonprofit service provider with over 20 different service lines. We manage residential group home program, summer camps for children with IDD and autism, find employment for people with mental and psychical disabilities, afterschool programs, adoption and foster services, crisis services, among many others. One of our service lines is our Assertive Community Treatment Team (ACTT).

Assertive Community Treatment (ACT) is a full spectrum, long-term, community-based treatment model for adults with severe and persistent mental health needs that have not been adequately treated by more traditional treatment services.

Team members help the person address every aspect of their life, whether it be medication, therapy, social support, employment or housing.

Assertive Community Treatment is an evidence-based, treatment approach that involves a high level of coordinated care and supports men and women to manage their mental health challenges and live independently.

Position Summary:

This member of the multidisciplinary team has a responsibility to carry out rehabilitation and support functions and assist in treatment; substance abuse services; education, support, and consultation to families; and crisis intervention under the clinical supervision of staff with professional degrees.

Essential Job Functions:

  • Provides case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team.
  • Assumes primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the Individualized Treatment Team (ITT);
  • Provides individual supportive therapy and system management.
  • Ensures immediate changes are made in treatment plans as client’s needs change.
  • Educates and supports client’s families; and advocates for clients’ rights and preferences.
  • Participates in comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g. self-care, living situation, nutrition, money management); and family structure and relationships.
  • Consults with community agencies and families to maintain coordination in the treatment process.
  • Provides on-call intervention covering nighttime hours and serves as a backup to evening and weekend staff.
  • Documents client progress to maintain a permanent record of client activity according to established methods and procedures.
  • Participates in daily staff organizational meetings and treatment planning review meetings.
  • Participates in providing substance use and treatment services.

  • Provides ongoing assessment of clients’ mental illness symptoms and clients’ response to treatment. Makes appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior which put clients at risk (e.g., suicidality).
  • Provides symptom education to enable clients to identify their mental illness symptoms. psychosocial rehabilitative interventions and care coordination
  • Provides direct clinical services to client son an individual, group, and family basis in the office and in community settings to teach behavioral symptom-management techniques to alleviate and manage symptoms not reduced by medication and to promote personal growth and development by assisting clients to adapt to and cope with internal and external stresses.
  • Provides multiple-stage individual and group treatment in the office and in community settings to develop a trusting relationship with clients so that they may more freely discuss substance use and its effect on mental and physical health and daily functioning. May participate in setting treatment goals and plans with clients to reduce or abstain from substance use and to learn replacement behaviors.
  • Coordinates with outside inpatient services to detoxify clients and establish linkage to outpatient treatment, self-help programs (e.g., Alcoholics Anonymous, Narcotics Anonymous), outpatient services, and residential facilities.
  • Participates in the provision of rehabilitation services.

Supervisory Responsibility:

Qualified Professionals (QPs) may be required to provide supervision to a staff member of Associate Professionals (APs) and Paraprofessionals (PP) status.

 
Position Requirements

Minimum Qualifications:

  • Must have a strong commitment to the rights and ability of each person with a severe and persistent mental illness to live in normal community residence; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.
  • Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for client rights and personal preferences in treatment are essential.
  • Must have a valid, unrestricted, state appropriate driver’s license for the State of North Carolina.

Minimum Education Qualifications:

A bachelor’s degree in a related human service field with a minimum required 1 year full-time, post-baccalaureate accumulated Mental Health, Dual Diagnosed or Substance Abuse experience with the population served or the equivalent of education and required experience, as required in the service definitions
