Description

This position includes direct support and completing case management assignments in conjunction with our Designated Coordinators.

Be a part of creating meaningful experiences for individuals with a Developmental Disability. This position is at our Linden location in White Bear Lake where we focus on:

  • On-site recreation and leisure programs including pet and music therapy, art, gardening, sensory activities, etc.
  • Community activities including volunteering.

Enhance dignity and promote independence

  • Helping with personal care needs
  • Lunchtime routines
  • Medical routines
  • Passing medications

Continue providing direct care and support while you begin to develop new skills:

  • Take on more responsibility and further develop your communication and organizational skills.
  • Work with those who are just graduating from High School up to and including those who are thinking about transitioning to retirement.

Job duties would include:

  • Accurately complete all assigned paperwork and data within the designated time frame.
  • Develop, follow and revise documents including assessments and Individual Abuse Assessment and Prevention Plans.
  • Maintain and assure service-related data are complete, accurate and in compliance with 245D and other regulatory standards.
  • Ensure accurate daily charting for assigned individuals.
  • Participate in team meetings

Provide direct care and support service delivery by:

  • Uphold all protection and service-related rights
  • Support and encourage the individual’s independent living skills, such as: maintaining their appearance, health care needs, age-appropriate social skills, etc.
  • Follow Funds Management and Safekeeping Policy when accessing monies or property.

Job Coaching

  • Ensure the use of the least restrictive interventions necessary to support achievement of outcomes and personal goals.
  • Engage with and encourage active participation in the community, as well as with chosen PAI activities.

Position Requirements
  • Pass DHS background study and maintain clearance to provide direct contact services without continuous supervision
  • High school degree or G.E.D
  • Be at least 21 years of age
  • Demonstrate effective oral and written communication skills
  • Computer skills including Microsoft Word and ability to learn data entry program
  • Experience supporting persons with intellectual disabilities

Some positions require:

Reliable transportation for work driving purposes, valid driver’s license, approved driving record and proof of valid auto insurance

 
Preferred Requirements: •Day service program or residential experience with persons with intellectual
•Familiarity with 245D licensing requirements
