SAFY of Canton, Ohio is seeking a mission focused professional to serve as a full time Case Coordinator responsible for providing in-home support services for foster parents and children.

As a Case Coordinator you would:

  • Work independently and create your own schedule based on your caseload and availability of your assigned families. This includes potentially working until 8pm several nights a month, based on the needs of the families.
  • Provide in-home, direct services to families and children in your caseload. Generally, caseloads consist of 12-15 clients that you'll see weekly.
  • Participate in our, paid rotating on-call schedule.
  • Create documentation for all services you provide in our Electronic Record System. Documentation includes Assessments, Diagnosis, Treatment Plans and Progress Notes, and Monthly reports. Overall documentation time is about 30%.
  • Develop and maintain consultative and liaison relationships with members of the child’s treatment team (biological parents, school, SAFY team members and area county workers)
  • Travel to families homes, in and around the Canton area - mileage reimbursement included.
  • Overall hours being worked outside of the office are estimated to be around 60%, which includes traveling to and from visits and time spent conducting a home visit.
Position Requirements

What you bring to this role:

  • Bachelor’s degree in the field of social work, counseling, psychology, sociology, child & family studies, or other approved related field of human services
  • Required Licensure- LSW, LPC or License Eligible
  • Two years of relevant experience with above listed degrees, experience in child welfare preferred, OR relevant experience in working with children in general.
  • Two years of relevant experience required with related field of human service degrees
