Turning Point Communtiy Programs is hirng a Shelter Case Manager to work at our Hospitality House in Grass Valley. Summary: Responsible for assisting Hospitality House clients in meeting their expressed mental health goals while living in the community or transitioning from a more restrictive setting. Specific assistance with medication management, housing, counseling, support and advocacy will be provided as needed. Please note: The Shelter Case Manager works directly under the clinical supervision and direction of a Hospitality House Supervisor or Program Manager.

Resposibilities:

  • Maintain all client records and complete required documentation according to shelter standards. This may include, but is not limited to, data entry, progress notes, activity reports, and logs
  • Carries a client caseload of approximately 30 clients.
  • Create overall client case plans with major client input. Monitor client progress with plan activities
  • Advocates for clients in all areas of treatment and helps them apply for and receive services.
  • Advocates for and assists clients and their families in accessing and receiving mental health treatment, medical care, housing, food and clothing as indicated.
  • Help educate the clients on mental health and substance abuse issues and refer to other providers as necessary
  • Perform assessments of housing barriers and create individualized housing stabilization plan for sheltered and outreach individuals
  • Develops individual service plans for each client with their input whenever possible.
  • Must spend each workday engaging members in the field including meeting regularly with clients at Turning Point sites, at jobsites, homes, board and care facilities, skilled nursing facilities, psychiatric facilities, correctional facilities, court and other appropriate sites. Also includes transporting clients to appointments, activities and shopping as appropriate.
  • Provides direct crisis intervention on behalf of clients including, but not limited to telephone, walk-in services, and outreach services and after hour “on-call” coverage to program clients on a rotational basis, and completing 5150 assessments.
  • Assists clients with locating job opportunities and provides support for the member’s work experience, when applicable.
  • Locate available housing, negotiate with landlords, and assist clients with rental applications and interpreting lease/rental agreements
  • Develop and maintain positive relationships with local area landlords and property managers and develop and update a housing resource directory
  • Assess and inspect housing units for compliance with funding guidelines
  • Investigate and determine client eligibility for housing services, in addition to assisting with credit repair, addressing housing barriers, and accessing other services such as job training, counseling, medical services, income or benefits, etc.
  • Assists in establishing client’s eligibility for Medi-Cal or other benefits and advocates for continuation of benefits when appropriate.
  • Interacts productively within the team to provide holistic mental health and case management services to an identified group of clients.
  • Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
  • Monitors medication distribution and related charting
  • Perform client reviews, collect client data and input data into HMIS
  • Work with the fiscal staff to determine the amount of financial assistance available for rent, deposits, utilities, or other housing expenses.
  • Cooperate with the data and information needs of the Nevada County Continuum of Care Collaborative.
  • Drives on program business using personal vehicle.
  • Maintains clean driving record as well as currency of driver’s license and vehicle insurance/registration.
  • Adheres to and upholds the policies and procedures of Turning Point Community Programs.
  • Attends all team and agency staff meetings unless approval for non-attendance is secured by the Program Manager. Team Leader or the Program Director.
  • Meets the standards set for performance in all aspects of job duties.
  • Meets the standard for consistent attendance (not more than a 5% absence rate per month) by reporting to every scheduled shift on time and ready to work and by the judicious use of paid sick leave and avoiding any situations resulting in the need to dock pay for time not worked.
  • Check TPCP e-mail at least twice a week for important updates and notices.

Schedule: Wednesday-Friday 9:00 am - 5:00 pm, Saturday and Sunday 12:00 pm - 8:00 pm

Travel: Field visits; transports clients; MH court; off-site meetings/trainings

 

 
Position Requirements

Preferred Qualifications:

  • Bachelor’s Degree in Social Work or related field and four years varied experience as a provider of mental health services. 
  • Additional qualifying experience may be substituted for education.

Minimum Qualifications:

  • Associates Degree in Social Work or related field and a minimum of six (6) years varied experience as a provider of mental health services is desired. 
  • Additional qualifying experience may be substituted for education.
