Community Case Manager 

Description

The Community Case Manager (CCM) shall be responsible for supporting adults with disabilities of all ages and others who need assistance to lead self-directed lives and contribute to their communities, as well as support behaviors that enhance inclusion in their communities. People requiring support may also have co-occurring disorders such as, mental illnesses, substance abuse disorders, or medical conditions. The CCM responsibilities include, but are not limited to:

  • Teaching daily living;
  • Communication and work skills;
  • Providing safe transportation;
  • Assisting with or coordinating medical and other support services appointments;
  • Maintaining accurate and detailed documentation and records;
  • Ensuring that there is consistency in the supports provided to each person with disabilities, and that there is effective communication between team members;
  • Ensuring a holistic approach is provided to people receiving services;
  • Advocate for the rights of each person, as well as promote self-empowerment; and self confidence to individuals to advocate for themselves; and
  • Implementing the philosophy, policies, and procedures of OHI at all times

 
Position Requirements
  • Must have a bachelor’s degree in psychology, behavioral health, social work, special education, counseling, rehabilitation, or a closely related field
  • One year experience supporting individuals with intellectual disabilities
  • Must work as an effective member of a team and meet with all team members to monitor and assess a supported person’s Plan
  • Coordinate professional evaluations and service
  • Thorough and accurate documentation
  • Proven ability to meet deadlines
  • Have a flexible schedule
  • Conflict Resolution skills and Crisis Intervention
  • Have a valid Maine Driver’s License, own a vehicle, and must be authorized to transport individuals with disabilities
  • Will be required to run, squat, stoop, bend, kneel, climb stairs, occasionally lift 50 pounds or ¼ of body weight, and/or push/pull against resistance
  • Have or acquire within the first 30 days of employment, and maintain certifications thereafter:
    • Certified Residential Medication Aide (CRMA)
    • CPR/First Aid
    • PACT Certification

About the Organization OHI is a Maine-based, public nonprofit organization providing supports and services to people with intellectual disabilities, autism, and mental illness for over 43 years. With offices in the greater Bangor area, specifically in the towns of Bangor and Brewer, OHI currently supports about 400 people in six Maine counties. OHI provides 24-hour residential services and various community services for people to live productively in their communities.

OHI also provides permanent housing in Bangor and Brewer for adults with disabilities, and professional development training for people, including family members, caregivers and support workers. In November 2013, OHI opened the Brewer Area Food Pantry as both a means to meet a growing need and to give back to a community which the people it supports rely upon.
OHI strongly believes in providing person-specific intellectual disability and mental health services tailored to the specific needs and desires of the person supported.
