Service Coordinator/Case Manager

Position Summary: The Early Intervention program provides supports and services to children from birth to age three who are experiencing delays in development, showing atypical development, or who have a diagnosed condition that makes them more likely to experience delays. Early Intervention Service Coordinators are responsible for coordinating services, ensuring the success of Individualized Family Service Plan (IFSP) teams, assisting with transition planning and serving as a resource for caregivers.

Essential Job Functions:

Provide services to clients

  • Assist families in the development of an Individualized Family Service Plan based on family concerns and priorities
  • Assist families in developing outcomes and identifying appropriate Early Intervention services
  • Assist with the coordination of Early Intervention supports and services
  • Monitor service delivery and family satisfaction with Early Intervention services
  • Assist with referrals to appropriate childcare providers and other community resources as needed
  • Provide families with developmental information and activities
  • Assist families in transitioning from the Early Intervention program to other supports and services

Documentation of Services

  • Submit case notes for home visits/ face to face visits according to schedule
  • Document all contacts on behalf of the child/family in individual client log
  • Maintain accurate and organized client files that clearly document child progress
  • Become familiar with the Early Intervention Practice manual and adhere to the rules and regulations put forth by the Virginia Part C offices

Collaborate with program staff, the community, families, and other therapists

  • Facilitate IFSP meetings and ensure that all pertinent information is conferred to all team members, and that appropriate actions are being taken
  • Coordinate and attend transition conferences for Part B school services.
  • Attend departmental staff meetings as well as individual meetings with the Program Supervisor.
  • Attend professional training and workshops
  • Attend mandatory Early Intervention and trainings as requested (Kaleidoscope, monthly provider meeting)
Position Requirements

Minimum Qualifications:

  • Bachelor’s degree from accredited four-year college/university in one of the following: Allied Health, Early Childhood, Child and Family Studies, Special Education, Psychology, Music Therapy, Social Work, Human Services or related field.
  • Experience working with young children. Experience working with children in their natural environments (home, daycare) is preferred.
  • Knowledge of child development and recommended practices in service provision for children with or at risk for developmental or behavioral/emotional disabilities.
  • Ability to work independently with minimal supervision.
  • Outstanding oral and written communication skills including clarity and articulation of thought, appropriate grammar, style, form and punctuation.
  • Completion of required Kaleidoscope training within 12 months of hire.
