Description

Serves as the Medical Director of the methadone program providing intake medical evaluations (inductions), dosage determination, and medication management including the supervision of withdrawal management (detoxification and withdrawal). Serves as the Medical Director of the ICF/IID, acting as the Primary Care Physician for all ICF/IID recipients. Serves as the Primary Care Physician for the Residential Crisis Stabilization Unit, providing intake medical evaluations within 24 hours of admission, responding by phone 24/7 to urgent medical issues, and maintaining and managing a pool of part-time physicians necessary to ensure coverage.

DISTINGUISHING ELEMENTS OF THE CLASS:

I. IMPACT: Work of this class has immediate effect on admissions to the methadone and residential crisis stabilization programs. The incumbent has the responsibility for assuring that clients are appropriately screened for methadone maintenance and/or withdrawal management according to federal guidelines and are medically cleared for residential treatment. The Physician has direct responsibility for the medical care and well-being of assigned individuals.

II. LATITUDE: The Senior Medical Director, the Medical Director of Crisis Stabilization Services and the Clinical Services Administrator for Opioid Replacement to provide consultation and supervision on an ongoing basis. Medical screenings are generally performed with a nursing assistant. Other duties are performed independently. The ICF/IID Medical Director consults with the nurse regarding ongoing medical complex diagnosis and orders diagnostic testing.

III. INTERPERSONAL CONTACTS: Work involves regular contact with individuals presenting with substance use disorders and their families, behavioral health diagnosis, co-occurring disorders and those with intellectual and developmental delays. Incumbent will also participate in regular case staffings with a variety of staff members.

V. DIFFICULTY: Work involves the frequent provision of general medical assessment and treatment, knowledge of withdrawal and overdose symptoms, use of methadone as medication assisted treatment, and long term care of individuals with complex medical care needs.

VI. WORKING CONDITIONS: Work is performed in multiple agency environments, including office-based, residential crisis stabilization program, and ICF settings. Incumbent will work with individuals with substance use disorders, primarily opioid, those presenting with behavioral health impairment or co-occurring disorders who may be physically unhealthy and emotionally unstable.

VII. KNOWLEDGE, SKILLS AND ABILITIES:

Thorough knowledge of the assessment of addiction in opiate addicts, including symptoms and signs of withdrawal and overdose.

Thorough knowledge of the application of MAT, primarily, methadone treatment.

Knowledge of behavioral health diagnosis and occurring disorders.

Knowledge of long term care medical and behavioral care.

Knowledge of Department of Health, DMAS, and ICF/IID regulations.

Ability to provide comprehensive medical examinations.

Ability to establish and maintain rapport patients with Mental Health and Substance use disorders.

Thorough knowledge of primary medical care.

Ability to establish effective working relationships with a variety of professionals.

Good knowledge of the laws, guidelines, and legal procedures relating to mental health care or substance abuse treatment to include HIPAA and 42 CFR Part 2.

VIII. LICENSURE: Unrestricted license as a physician by the Virginia State Board of Medicine and a current DEA license. ABMS certified or certified eligible in a primary care specialty

IX. PRINCIPLE DUTIES AND RESPONSIBILITIES:

Provides physical examinations as required by federal and state standards and program guidelines.

Maintains up-to-date, well documented medical records which comply with federal and state standards.

Prescribes methadone and medication management including withdrawal management (supervision of detoxification and withdrawal).

Prescribes medications to address immediate health care needs in crisis stabilization/ambulatory detox setting.

Prescribes medications to address long term health care needs.

Reviews all laboratory results and makes appropriate recommendations.

Assumes on call responsibilities for admission dosages or other program concerns of a medical nature.

Participates in case staffings as a member of a multidisciplinary team.

Counsels clients on medical issues.

Other duties as necessary.

X. PHYSICAL REQUIREMENTS: Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment, such as, but not limited to a, personal computer, calculator, copier, and fax machine. No significant walking, moving, carrying, climbing, bending, kneeling, crawling, reaching, and handling, sitting, standing, pushing, and pulling.
