What are we looking for in a Case Manager CDP?

Working with a multidisciplinary team, Case Manager CDPs provide supportive counseling, case management, and chemical dependency treatment to clients within designated intensive outpatient programs. Using motivational interviewing and excellent communication skills, Case Manager CDPs will engage clients in treatment early and assist the client in achieving treatment goals. This position is best suited for someone who is organized, can build rapport quickly, and is knowledgeable about recovery principles.

This position is part of our Program for Assertive Community Treatment (PACT) team. PACT is a service delivery model with the primary goal of recovery through community treatment. Its target population is those with challenging and persistent mental health issues. The PACT model is characterized by a team approach, shared caseload, time unlimited services, flexible service delivery and 24/7 crisis availability. Schedule: Monday-Friday 8am-5pm, plus participation in the on-call rotation for after-hours crisis coverage.

Case Manager CDP duties may include:

  • completing chemical dependency assessments
  • facilitating behavioral health treatment & education groups
  • collaborating with the client and the treatment team to develop an individualized treatment plan that reflects the client’s goals & needs
  • coordinating with other treatment teams & community providers to expedite treatment for each client and advocating for clients to attain services & benefits, as appropriate
  • completing all progress notes and documentation in a respectful, client-centered manner
  • participation in on-call rotation for after-hours crisis coverage
Position Requirements

Education/Experience: Bachelor's degree in social services or related field and current Chemical Dependency Professional or Trainee certification in Washington

Professional Credentials: Eligible for Agency Affiliated Counselor registration

Other Credentials Required: Driver's license, vehicle liability insurance
