We provide support services and advocacy to 400 adults and children and their families in Harford and Cecil Counties. The Arc supports people with intellectual and developmental disabilities to create lives that mirror yours and mine: students attending neighborhood schools, adults going to work and living in their own homes, and individuals enjoying the social and recreational opportunities in their communities. We are a private, non-profit local chapter of The Arc of Maryland and The Arc of The United States, the largest volunteer organization in the world devoted exclusively to improving the quality of life for all adults and children with developmental disabilities.

Description

Position Summary:

The Community Living Case Coordinator is responsible for ensuring the supported individuals are working toward their individual goals. They assist the individuals in developing appropriate outcomes, goals and strategies and manage the individual planning process, coordinating information and the efforts of internal and external members of the individual's team.

Position Responsibilities:

In a manner consistent with supporting The Arc NCR Mission Statement the Community Living Case Coordinator shall do the following:

Quality Supports:

  • Ensures individuals served live the life they want the way they want as outlined in our philosophy.
  • Develops a relationship with the individuals served that is respectful, based on mutual trust and that maintains professional boundaries.
  • Serves as the central point of contact for the Service Coordinators of the individuals on his/her case load.
  • Develops appropriate outcomes, goals and strategies in collaboration with the Community Living Supervisor for each individual's IP meeting and submits for internal approval at least 5 weeks prior to the scheduled meeting.
  • Sends medical information and approved outcomes, goals and strategies to Service Coordinators.
  • Enters all individuals new to receiving Community Living Services and all IP data into the program database and generates outcome reports and data collection sheets.
  • Monitors and records clients' progress to ensure that goals and objectives are met.
  • Completes periodic reviews for each individual when required.
  • Ensures IP books in the main building and the houses are current and meet organizational expectations.
  • Ensures that the individual's plan is carried out as outlined in the written plan, programs, schedules and other documentation.
  • Provides advocacy for individuals served based on their personal choices.
  • Ensures all individuals served are living according to their rights and responsibilities and are free from abuse and neglect.

Quality Team Members:

  • Trains supported staff providing services from other departments on individual's IP.

Financial Excellence:

  • Completes, maintains and submits required reports and documentation in a timely manner.
  • Schedules and coordinates the attendance of all necessary parties for all meetings for individuals to include but not be limited to: annual individual planning meetings, emergency meetings, interim meetings for individuals receiving services from the Community Living program.

Other duties as assigned.

 
Position Requirements

Education, Training and Experience:

  • High School Diploma, Required
  • Bachelor’s Degree in Human Services or related field, preferred
  • Three (3) years of experience in a Case Coordination or Management position
  • Fluent in Microsoft Office Suite
  • Experience with database systems

Other Requirements

  • Access to reliable transportation
  • Active valid driver’s license and driving record that has no more than two (2) points.
  • Maintenance of all required trainings under DDA guidelines and The Arc Northern Chesapeake Region policies and procedures
  • Flexibility of scheduling to meet agency and program needs
