About the Organization Hill Country Mental Health and Intellectual and Developmental Disabilities Centers is a unit of local government and a not-for-profit, 501C provider of Mental Health and Intellectual Developmental Disabilities, Substance Abuse, and Veteran Peer Services throughout the greater Texas Hill Country Region. Hill Country MHDD Centers is designated to be the Mental Health Authority by the Texas Department of State Health Services (DSHS) and the Intellectual and Developmental Disability Authority by the Department of Aging and Disability Services (DADS) for a 19 County Service Area including: Bandera, Blanco, Comal, Edwards, Gillespie, Hays, Kendall, Kerr, Kimble, Kinney, Llano, Mason, Medina, Menard, Real, Schleicher, Sutton, Uvalde and Val Verde Counties. Job Title: Care Coordinator Adult/Child - QMHP-CS

Location: Gillespie County Mental Health Center

Essential Duties and Responsibilities

This exciting and innovative team is seeking a Care Coordinator to work with both adult and children populations. The Care Coordinator is responsible for providing services and supports to individuals with mental illness to assist the individual in reaching their vision of recovery. The position will assess the individual’s strengths and needs and in collaboration with the individual develop a person-centered recovery plan. The Care Coordinator will provide skills training; linkage and referral; crisis prevention/management, Supported Housing and Supported Employment services. Additional responsibilities also include scheduling and carrying out individual and group rehabilitation services and case management using evidenced based curriculum. This position will be required to participate in a crisis on-call rotation twice a month and be available for face-face assessment within 1 hour of notification on assigned days. This position must have flexible hours to accommodate individuals who cannot meet between 8 and 5.

**After meeting the performance standards two consecutive quarters, the Care Coordinator is eligible for a pay increase.

Position Requirements

Minimum Qualifications:

  • Bachelor’s Degree from an accredited college or university with at least 30 hours of course credit work in the following areas of study: Counseling, Education Psychology, Gerontology, Human Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Work, Sociology
  • Position requires candidate to meet QMHP Credentialing Guidelines

  • Lives within thirty (30) miles of Center with reliable transportation

Preferred Qualifications:

  • Bilingual: English/Spanish Speaking
  • Three to five years of experience working in a mental health setting providing community-based services within the community.
  • Experience working in a team setting
  • Computer proficient - Knowledgeable with Microsoft Office Products

  • Automobile Insurance (Current without lapse)

Compensation $16.34 per hour / $34,000 - After meeting the performance standards two consecutive quarters, the position is eligible for a pay increase.  
On-Call Pay Schedule (If applicable) $40.00 Per Day on a rotating basis.
