Case Manager - multiple positions 


What are we looking for in a Case Manager?

Case Managers work within various teams at Comprehensive to provide support and coordination of care to our outpatient clients. Recognizing the complex needs beyond mental health and substance use treatment, Case Managers work with clients to build skills necessary for independent living, self-advocacy, treatment engagement, and social participation. Case Managers support clients and ensure they are connected with the necessary community resources that will enable them to fully engage with and be successful in treatment. To be successful in this role, Case Managers will need excellent communication and relationship-building skills, knowledge of community resources, and comfort with making home visits and traveling locally throughout the workday.

Case Manager duties may include:

  • collaborating with each client to create an individualized treatment plan
  • assisting clients with obtaining benefits and community services
  • serving as protective payee for assigned clients and assisting with budgeting and tracking spending
  • visiting a client in their home or apartment to deliver medications, check on safety issues, or assist with life skill development
  • coordinating with other service providers and treatment teams to expedite services and advocate on the client’s behalf
  • monitoring clients’ progress, symptoms, and medication response
  • facilitating educational, behavioral health, or skill-building groups
  • completing all documentation in a respectful, client-centered manner

We currently have six Case Manager positions available on various teams throughout our Yakima location:

Corrections: This position will coordinate with Yakima County Department of Corrections to provide case management services to clients residing within the Yakima County Jail. The Case Manager must be comfortable working with inmates in a corrections setting. Fluency in English & Spanish is required. Schedule: Monday-Friday day shift.

Intensive Community Support - Behavioral Health Diversion: In this position, Case Managers provide intensive community support to individuals who have been identified as high risk for incarceration, to those who are not engaged or minimally engaged with services, and to those who are being released from a corrections facility or who have been referred to the Behavioral Health Diversion Program. The goal of this position is to connect high-risk individuals with community-based treatment that enables clients to be proactive with their illness management and recovery, thereby reducing interactions with law enforcement and correctional facilities. Schedule: Monday-Friday day shift.

PACT: Program for Assertive Community Treatment (PACT) is a service delivery model with the primary goal of recovery through community treatment. Its target population is those with challenging and persistent mental health issues. The PACT model is characterized by a team approach, shared caseload, time unlimited services, flexible service delivery and 24/7 crisis availability. Schedule: Monday-Thursday 8am-7pm, plus rotating on-call shifts for crisis support.

Rehab: The Rehab team provides services to clients who experience severe, persistent mental illness and who need a wider scope of care than can be provided by short-term interventions. Case Managers provide support and assistance to clients of all ages. Schedule: Monday-Friday day shift.

Sunrise Club: The Case Manager will provide daytime support to members at the Sunrise Club in Yakima. The Sunrise Club follows the clubhouse model of psychosocial rehabilitation and provides members with meaningful opportunities for socialization, employment training, skills development, and multi-faceted recovery. Schedule: Monday-Friday day shift.

Position Requirements

Education/Experience: Bachelor's degree in social services or related field. Some positions require bilingual English / Spanish.

Professional Credentials: Eligible for Agency Affiliated Counselor registration in WA

Other Credentials Required: Driver's License, Vehicle Liability Insurance
