Hospital Liaison 


What are we looking for in a Hospital Liaison?

The primary responsibility of the Hospital Liaison is to ensure that clients who are being discharged from hospitals or inpatient care transition smoothly into community-based care. The Hospital Liaison collaborates with various psychiatric facilities to identify appropriate placements and participate in discharge planning for each client. This role also assists in making referrals to community providers and coordinates with treatment teams to ensure a smooth transition between levels of care. This position will require strong organizational and communication skills, a desire to work closely with multiple organizations, and knowledge of community programs and service providers.

Hospital Liaison duties may include:

  • providing a combination of direct services and service coordination to assist clients as they discharge from inpatient services
  • arranging services for new clients awaiting placement on a treatment team, such as appointments with physician or nurse, crisis bed placement, etc.
  • monitoring bed status at Eastern State Hospital and Bridges, and sharing this bed and discharge information at daily Acute Care meetings
  • reviewing documentation to determine placement needs and requesting additional assessments as needed to determine appropriate level of care
  • monitoring LRA clients until they can be seen for intake or assigned to a treatment team, and completing LRA monthly renewals when needed
  • assisting new clients by arranging transportation, delivering medications, helping to apply for social service benefits, and acting as liaison with other treatment agencies

Position Requirements

Education/Experience: Bachelor's degree in social services or related field and two years of related experience; or equivalent combination of education and experience.

Professional Credentials: Eligible for Washington State Agency Affiliated Counselor Registration

Other Credentials Required: Driver's License, Vehicle Liability Insurance
