Healthy Transition Youth and Young Adult Advocate

OVERVIEW:

The Healthy Transitions Youth and Young Adult Advocate will work to develop solutions to resolve homelessness, educational needs, training/employment needs, mental health and substance abuse needs. Each client will receive individualized plans using a wraparound approach.

POSITION STATUS: Full-Time – Grant Funded

SALARY LEVEL: DOE

WORK SCHEDULE: Monday thru Friday schedule Varies

ESSENTIAL FUNCTIONS OF THE JOB:

  • Under the direction of the Healthy Transition Project Manager, the Healthy Transitions Youth and Young Adult Advocate will provide direct support to clients on an individualized basis.
  • Participate in all administrative team meetings, discussions, implement the day-to-day activities to spread awareness;
  • Find creative ways to promote the Healthy Transitions Program;
  • Provide support to youth/young adults involved with the Healthy Transitions Program and wraparound services through assistance in supporting authentic youth/young adult voice in individual treatment, service delivery, system design, and governance throughout Healthy Transitions;
  • Support youth/young adults in developing and coordinating youth/young adults’ leadership training opportunities, including conferences;
  • Partnering with Youth and Young Adults:
  • Develop youth support/focus groups;
  • Develop and coordinate culturally competent policies designed to amplify youth/young adults’ voices within Healthy Transitions and partner organization
  • Identify barriers to effective youth/young adults’ voice solutions
  • Manage and prioritize a moderate youth and young adult caseload.
  • Collaborate with Housing Resource Center staff and local landlords to secure housing
  • Conduct housing quality inspections to ensure housing meets standard
  • Develop and maintain effective communication with area landlords and housing providers
  • Make an in-depth, individual needs assessment with each participant
  • In conjunction with clients and other agencies with which they are involved, develop and monitor individualized program action plans, which include action steps, goals and timelines
  • On a recurring basis communicate with each client to review progress made on action plans.
  • Utilize trauma informed techniques, motivational interviewing and progressive engagement. Work with client to design individualized safety plan relevant to present and future.
  • Coordinate linkages with other agencies with which a client is involved, facilitate fluid service delivery and maintain collaborative program planning and service coordination. Establish interagency staffing meetings as needed or desired by the client.
  • Act as a resource and referral source for clients, providing in-person linkages when able.
  • Facilitate client access to other housing resources if not transitioning in place in order to obtain permanent housing.
  • Develop a discharge plan with clients before they exit the program.
  • Maintain case files, grant required data and document services provided
  • Maintain current and closed files for data collection, statistical, and reporting purposes.
  • Compile statistics and generates reports as required.
  • Keep Project Manager informed of daily operations of the program.
  • Performs other duties as assigned

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED BY THE POSITION

  • Ability to maintain a cordial relationship with various departments of the organization for proper coordination as well with grantees/funding agencies
  • Must be well organized and have detail oriented approach
  • Proficient in the use of MS Office (Word, Excel, etc.), internet, fax, copier, printers, scanners and telephone
  • Ability and willingness to learn new systems, techniques and procedures
  • Excellent organizational skills.
  • Excellent written and oral communication skills
  • Ability to work independently and complete tasks on time.
  • Ability to maintain confidentiality

DESIRED QUALIFICATIONS

  • High School Diploma or GED;
  • Strong commitment to work with homeless youth and young adults
  • Lived experience in homelessness, obtaining educational goals, unemployment/lack of training, mental health, and/or substance abuse
  • Public speaking experience;
  • First Aid Mental Health Certification or ability/willingness to obtain within 6 months;
  • Paratransit training or ability/willingness to obtain within 6 months;
  • Due to extensive travel and outreach, fitness, ability to hike up to 3 miles, climb stairs, work in difficult weather, lift 25 pounds over head and physical ability to access difficult places is required
  • Experience working with Indian tribes in grant related activities;
  • Must be able to travel and work flexible schedule as needed;
  • Knowledge of the Lower Elwha Klallam Tribe and surrounding communities;
  • Knowledge and experience related to crisis intervention, LBGTQ, Native American youth and young adult homelessness, domestic violence, mental health, case management, positive parenting and substance abuse preferred.
  • Ability to be positive, enthusiastic and creative while coordinating a diverse group of individuals.
  • Ability to work well with people of all ages and cultural/economic backgrounds.
  • Ability to motivate others to participate in training, educational opportunities and other activities.
  • Ability to communicate clearly and effectively, verbally and in writing.
  • Ability to work effectively, either independently or cooperatively with others and to work a flex schedule based on the program needs.
  • Must pass UA testing prior to employment and submit random urinalysis upon request;
  • Ability to adhere to LEKT Employee Handbook
  • Valid Driver’s License.

OTHER CONSIDERATIONS

  • Must be able to comply with the Lower Elwha Klallam Tribe’s Drug Free Workplace policies and personnel policies.
  • Other duties as assigned relevant to job.

As with all positions of the Lower Elwha Klallam Tribe, this position is Indian Preference in hiring in Accordance with P.L. 93- 638.
