TMS Peer Support Specialist

Position Summary:
The Transition Management Services Peer Support Specialist provides services to individuals participating in the Transition to Community Living Initiative (TCLI). TMS Peer Support Specialist provides rehabilitation service intended to increase and restore an individual’s ability to live successfully in the community by maintaining tenancy. The TMS Peer Support Specialist focuses on increasing the individual’s ability to live as independently as possible, managing the illness, and reestablishing his or her community roles related to the following life domains: emotional, social, safety, housing, medical and health, educational, vocational, and legal.
Essential Job Functions:
  • 1. Provides coaching, mentoring, and consultation to the individual served to promote recovery, self- advocacy, and self-direction.
  • 2. Promotes wellness management strategies, which includes delivering manualized interventions (e.g., Wellness Recovery Action Planning or Illness Management and Recovery) and tenancy support services.
  • 3. Models recovery values, attitudes, beliefs, and personal action to encourage wellness and resilience.
  • 4. Provides consultation to team members to assist in understanding of recovery and the role of the Peer Support Specialist, promoting a culture in which Individuals’ points of view and preferences are recognized, understood, respected, and integrated into treatment.
  • 5. Serves as an active member of the Tenancy Support Team, equivalent to other team members, which includes assisting with the Person Centered Planning process.
  • 6. Supports and empowers the individual to exercise his or her legal rights within the community.
  • 7. Provides tenancy support services interventions and case coordination as needed.
  • 8. Participates in the initial development, implementation and on-going revisions of the Individuals’ Person Centered Plan (PCP), Crisis Plan, and Housing Support Crisis Plan
  • 9. Participates in “first responder” coverage and crisis response with the LME-MCO, provide crisis support to the individual by the initiation of WRAP or other crisis support plan. When required makes immediate telephone contact with the individual and a face-to-face within 90 minutes after a crisis has occurred.
  • 10. Participates in the provision and provides oversight to ensure structured rehabilitative interventions to individuals served such as Assessment, Individual Housing & Tenancy Sustaining Services, Money Management and Entitlements, Activities of Daily Living, Personal Health, Wellness and Recovery, Promote Community Integration
  • 11. Prepares and submits documentation accurately and on time.
  • 12. Participates in peer review.
  • 13. Completes required training within prescribed timeframes.
  • 14. Successfully participates in the DMH approved IPS training and provide services according to the service definition and IPS Fidelity Model.
  • 15. Other duties as assigned.

Position Requirements
Minimum Qualifications:
  • 1. A minimum of a high school diploma or equivalency required from an approved, accredited institution.
  • 2. Must be a NC Certified Peer Support Specialist.
  • 3. “Lived experience” and a personal recovery story specific to primary mental illness or substance use disorders.
  • 4. Have the knowledge, skills, and abilities to successfully provide TST to adults with mental illness and substance use disorders
  • 5. Minimum of two years’ experience working with the Adult Mental Health population.
  • 6. Excellent oral and written communication skills.
  • 7. Ability to handle multiple responsibilities and assign priorities.
  • 8. Must have current, unrestricted, state-appropriate driver’s license.
  • 9. Must have current, unrestricted vehicle insurance and provide own transportation for use in fulfilling job requirements.
