Our Child Placement Services program has an immediate opportunity for a Part-Time Child Placement Coordinator to join their team. This position will support Greenville, NC, and surrounding counties. Flexible hours (25hrs a week) with rotating 1x Month On-Call coverage required. Must be available to work evenings and weekends to accommodate client schedules.

Position Summary:

The Child Placement Coordinator functions primarily as coordinator of placement services for children receiving treatment foster care. Responsibilities include intake, casework, service planning, and planning/coordinating the services and resources affecting children and their families.

Essential Job Functions:

  • Advocates for personal outcomes for children and families, adhering to best practices.
  • Coordinates referrals for placements and receives all necessary paperwork prior to service delivery.
  • Coordinates admission and discharge planning with respective contracted agencies including completion of required documents prior to placement and conducts an initial visit with the child within 72 hours.
  • Supervises and monitors service delivery through in-home community-based visits assuring services meet or exceed state, local and ESUCP standards.
  • Assists the Licensing Coordinator with recruitment and provision of training of prospective foster parents.
  • Supports children, families, foster parents and contract agencies by responding to calls promptly.
  • Communicates with collateral agency representatives regarding child/family status including medication changes, residential situation, staff changes, school performance, incidents, etc. (within 24 hours)
  • Assures adherence to Client Rights and Confidentiality Rules including HIPAA regulations.
  • Attends community appointments as needed.
  • Provides direct supervision and support to Therapeutic Foster Families. Visits each Treatment Family weekly to provide face to face supervision/guidance and to oversee treatment.
  • Provides additional required training to Treatment Parents prior to placing an Intensive Alternative Family Treatment level child in the home.
  • Makes daily contact with Treatment Parents for Intensive Alternative Family Treatment level children.
  • Prepares and maintains service records on children and families in accordance with NC DSS Administrative Rule 10A NC 70 and DMA documentation standards.
  • Reviews documentation to ensure that all goals/objectives as outlined in service plan are worked on and documentation meets Medicaid guidelines.
  • Completes paperwork accurately and timely and files it by the 10th of each month for the month prior.
  • Ensures all required electronic data is complete, correct, and entered into Client Care Web within required timelines for Intensive Alternative Family Treatment level children.
  • Coordinates and oversees initial and ongoing assessment activities.
  • Facilitates person-centered planning and child & family team meetings. Develops initial and ongoing revisions of the person-centered plan and monitors implementation of the person-centered plan.
  • Links children/adolescents/families to additional services and arrange for referrals.
  • Completes application forms to access needed services in the community.
  • Assures authorizations and re-authorizations are submitted accurately by due dates.
  • Documents services according to ESUCP, state and federal guidelines.
  • Keeps supervisor informed of clinical and administrative issues relating to service delivery.
  • Participates in monthly on-call rotation for routine and emergency situations. Responds to emergency/crisis situations as needed during the day, night, holidays and weekends.
  • Assures that all required billing documentation is prepared accurately and forwarded by the due date to the appropriate agency/department.
  • Assists program in maintaining sustainability by following protocols for staying within budget.
  • Transports individuals being served, as needed.
  • Troubleshoots for treatment teams when issues or challenges arise.
  • Ensures incident reports are completed and filed within the 72-hour deadline.
  • Attends and participates in required trainings, seminars, workshops.
  • Presents to community stakeholders as needed to promote and grow the program.
  • Maintains a caseload of 6-8 TFC children and families.
  • Must demonstrate Easter Seals valued behaviors of Accountability, Commitment and Caring as appropriate to the position.
  • Completes required training within prescribed timeframes.

Supervisory Responsibility:

  • Supervises and monitors the documentation requirements for Therapeutic Foster Parents.
  • Provides weekly clinical oversight and supervision to Therapeutic Foster Parents.
 
Position Requirements

Minimum Qualifications:

  • Bachelor's degree in Social Work or related human services field from a college or university accredited by an entity approved by USDOE or CHEA.
  • Minimum of 2 years of experience working with children and families in a related field. (Directly related experience is defined as human services experience in the areas of case management, assessment, and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.)
  • Must have excellent verbal and written communication skills. Prior case documentation is a plus.
  • Must be comfortable working independently with strong time management skills and accountability.
  • Valid unrestricted, state appropriate driver’s license and ability to be insured by Easter Seals UCP’s insurance carrier.
  • Work is office/community-based; travel and work within any setting is required, including the private homes of persons served.
