Registered Nurse to support the health and medical needs of people using services of the NC START Central program primarily focused on guests admitted to the Resource Center. The Resource Center is a therapeutic 24/7 center that has four beds for people with IDD and MH needs.
Essential Job Functions:
Assessment of guests for appropriateness of admission to the Resource Center.
Participation in team calls for after-hours crisis; planned Resource Center stays and Crisis Resource Center Triage.
Manage the medication system by:
Working with the Administrative Assistant to coordinate medication orders and ensure accuracy for guests prior to admission to the Resource Center.
Work with Resource Center Counselors to check medications and verify orders at admission, transcribe orders and develop Medication Administration Records.
Train all staff in Medication Administration at employment and provide annual updates. Provide any additional training in medication administration as needed.
Collaborate on and assess the use of prn medications.
Provide oversight for administration of medications done by Resource Center Counselors on a quarterly basis or more often if indicated.
Provide medication administration when ordered route of administration is subcutaneous or intramuscular or otherwise requires the expertise of a licensed health care professional.
Provide oversight and training for staff in using medical equipment and in management of health conditions (e. g., CPAP, EpiPen, Nebulizers, Inhalers, Seizure recognition and management including use of Diastat and a VNS, blood glucose checks).
Ensure the Medication Room is cleaned and properly maintained, including having needed supplies and equipment for administration of medications.
Review Incident Reports for medication errors and problem solve prevention strategies.
Assess all guests for health issues and needs at admission to the Resource Center, including obtaining vital signs and determining BMI.
Monitor health issues and conditions for all guests during Resource Center stay.
Document on Nursing Progress notes as appropriate for all guests during Resource Center stays.
Support guests in Planned stays by:
Working with the assigned NCSTART Coordinator to get health updates and information about current health issues.
Review CCNC records for recent medical visits and health issues.
Provide support to home teams if follow-up for health issues is identified during Resource Center stays.
Support guests in Crisis stays by:
Completing MEDS assessments, in conjunction with Lead Resource Center Counselor, for guests in Crisis stays.
Working with the guest, family/guardian and coordinator to complete the HRST.
Completing the health and medical/psychiatric portion of Discharge Summaries.
Participating in Discharge calls and meetings.
Provide training for Resource Center Counselors and other staff regarding health promotion and education for guests using the Resource Center.
Train staff in Universal Precautions and assist in Blood Borne Pathogens exposure evaluation and treatment needs.
Assist Resource Center Management in evaluating staff injuries and need for further medical care.
Assist with monitoring the Resource Center for sanitation issues.
Develop and maintain relationships with health care providers, including physicians, hospitals and pharmacies throughout the region.
Attend all required meetings of the Clinical Team and Resource Center.
Teach CPR/FA/AED for all staff.
