Description

JOB SUMMARY: Under the supervision of the Program Director, the Housing Support Specialist serves as a member of the residential program, providing therapeutic skills development and support to the residents of the Supportive Housing Program.

ESSENTIAL FUNCTIONS:

  1. Contribute to the development, implementation, monitoring, and updating of the individualized rehabilitation plan
  2. Perform therapeutic rehabilitative skill development training to promote community integration by improving functional, social, interpersonal, problem-solving, coping, and communication skills in the areas agreed upon by the recipient and identified in the rehabilitative recovery plan and documents accurate/current recipient services in accordance with CSS standards.
  3. Provide Illness Management and Recovery (IMR) training and support in symptom monitoring, medication management, education on mental illness, relapse identification and prevention, Wellness and Recovery principles, etc.
  4. Provide Evidenced Based Practices – Motivational enhancement, Behavior Modification and Cognitive-Behavioral Techniques.
  5. Provide Crisis Intervention in emergent situations. (Bachelor’s level and above only).
  6. Coordinate and manage services. (Bachelor’s level and above only).
  7. Maintains the security and safety of the residents and the facility.
  8. Communicate, in written & verbal form, assessments & observations of consumers to other treatment team members.
  9. Supervise and monitor residents in self-medication
  10. Maintains accurate/current documentation according to CSS standards.
  11. Other duties as deemed necessary by the Program coordinator, Supervisor or Director.
 
Position Requirements

KNOWLEDGE/SKILLS/ABILITIES: Basic understanding of support services provided to service recipients as defined in Community Residences for Adults with Mental Illnesses - N.J.A.C. 10:37A and Community Support Services - N.J.A.C. 10:37B

SUPERVISORY REQUIREMENTS: None

MINIMUM QUALIFICATIONS: Bachelor’s degree from an accredited college or university in human services, psychology, social work discipline or a combination of the following: one or more years of college, plus related work experience which equals 4 years, OR HS diploma or equivalent + 4 years of related work experience. (Bachelor’s degree preferred)

LICENSES/CERTIFICATIONS NEEDED: Valid Driver’s License

MANDATED PROGRAM REQUIREMENTS: Community Support Services Training required. Completion of competency based training program for individuals providing adult mental health services; Certification in CPR/First Aid/CPI issued by a certified trainer; Completion of Relias Learning online trainings.
