What You Would Be Doing

The clinician/social worker reports to the clinical manager and is responsible for ensuring that the medically related emotional and social needs of the clients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state, and local standards, guidelines, and regulations.

Essential Functions

  • Develops a comprehensive social history and psychosocial assessment that includes the client’s problems, strengths, and preferences; including cultural, religious, and ethnic background and implications for the care plan.
  • Helps clients and families identify needs and develop a service plan to meet those needs.
  • Orients clients and families to the program, its services, its service limitations, and clients’ rights.
  • Involves clients and families in treatment planning, encouraging their attendance at care conferences or with outside providers/professionals.
  • Attends scheduled care conferences and identifies and interprets psychosocial needs of clients for inclusion in the care plan.
  • Completes mandatory documentation within timelines established by applicable regulations and policies.
  • Mediates issues that arise among clients, families, and staff.
  • Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, etc.).
  • Provides clients and families with crisis management services as needed.
  • Develops a comprehensive discharge plan in collaboration with clients, clients’ families, multidisciplinary staff, and referral agencies.
  • Trains and consults with staff regarding psychosocial needs that encompass mental health, development, and diversity of individual clients to increase staff knowledge and skills.
  • Educates staff about any relevant state legislation or regulations relating to behavioral health services.
  • Advocates on a case, policy, and program level.
  • Participates in policy decision making in a way that encompasses an understanding of the issues and implications relevant to the developmental and behavioral health needs of clients.
  • Meet required productivity requirements of 25 billable hours per week.
  • Provide clinical review and audit of clients’ charts and documentation to ensure compliance and support quality improvement.

Supervisory Responsibility

This position has no supervisory responsibilities; however, this position is required to have weekly or biweekly supervision with the Clinical Manager.

Work Environment

This job operates in a professional office environment, but regularly works in the community providing services outside of the office. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Physical Demands

While performing the duties of this job, the employee is regularly required to talk and listen. The employee frequently is required to stand; walk; drive; typing; speaking on the phone; sitting.

Position Type and Expected Hours of Work

This is a full-time position, working at least 40-45 hours per week. This position operates Monday through Friday during regular business hours; however there will often be times in which the requirement will include extending hours into 2-3 evenings a week. Occasional evening and weekend work may be required as job duties demand.

Travel

Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.

 
Position Requirements

Required Education and Experience

  • Bachelor’s or Master’s degree in social work or related field.
  • Licensed through the State of Ohio as a LSW/LPC/LISW/LPCC.

Preferred Education and Experience

  • One to two (1-2) years of supervised social work experience in a health care setting, working directly with individuals.
  • Experience working with people with autism spectrum disorders.
  • Experience working with people with mental health disorders.

Additional Eligibility Qualifications

  • Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff, and support agencies.
  • Must be able to relate to and work with ill, disabled, emotionally upset, and at times hostile clients/family members.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
