Service Coordinator

Our ever-growing Empower (Recreation & Transition to Work) Program has an immediate opportunity for a Service Coordinator to join their team in Elizabeth City. This is a Full Time position offering competitive salary and full benefits. Position Summary:

This position is responsible for supervision of developmental disabilities services for children (5-17yrs) and adults enrolled in the EMPOWER Recreation and Transition to Work programs. Responsible for ensuring best practices, supports based on the personal outcomes of the individual and or family, and compliance with all state, federal, and agency standards as required for assigned services. This position will be a train the trainer and will oversee staff training.

Essential Job Functions:

  • Ensures the appropriate development of person centered plans and the implementation of these plans.
  • Ensures the appropriate development of short-range goals and reports on progress towards those goals.
  • Monitors all required documentation to ensure local, state, federal and agency requirements are met.
  • Ensures quality services that support individuals to achieve personal outcomes. Protects the rights and ensures health, safety, and welfare of the individuals we support.
  • Collaborates with MCO (Managed Care Organization) and other entities to ensure supports are developed based on desires of individuals.
  • Works with individuals and their family to develop schedule for appropriate services.
  • Attends Care Coordination meetings at the request of families.
  • Ensures trainings are in place to maximize staff development.
  • Supports Empower management by ensuring necessary authorizations are in place before supports begin or continue ensuring that direct care professionals are working appropriate schedules.
  • Provides and coordinates training and ensures all training requirements for direct care professional are met.
  • Ensures staff and children’s records are complete and in compliance with requirements.
  • Conducts internal audits and facilitates Peer Reviews.
  • Follows and assures compliance with all Easter Seals Policies and Procedures.
  • Demonstrates Easter Seals valued behaviors of Accountability, Commitment and Caring as appropriate to position.
  • Protects the rights of those persons served and supports the right to dignity, humane care and freedom from abuse, neglect, exploitation and mistreatment.
  • Maintains regular attendance and punctuality, manages use of PTO time, and follows all policies and procedures regarding time off.
  • Ensures completion of required training within prescribed timeframes for self.
Position Requirements

Minimum Qualifications:

  • Bachelor's degree from a college or university accredited by an entity approved by USDOE or CHEA.
  • Minimum of Two (2) years’ experience working with children ages 5 yrs+ and adults with Intellectual and / or Developmental Disabilities, and meets qualifications for Qualified Professional.
  • Minimum 1 year experience working with MCO (Managed Care Orgnanization) to support individual's services as a private provider.
  • Must have a valid, unrestricted, state appropriate driver’s license and personal vehicle insurance.
  • Satisfactory criminal background check and acceptable motor vehicle record.
