Position Summary:

The ACT team psychiatric nurse practitioner (PNP) shall:

  • have an unrestricted license to practice as a registered nurse in North Carolina, have successfully completed a Psychiatric Mental Health Nurse Practitioner education program accredited by a national accrediting body, and maintain certification as PMHNP by a national credentialing body.
  • meet the requirements as specified in 21 NCAC 36.0800, approval and practice parameters for nurse practitioners, with at least three years full-time experience treating individuals with SPMI. The NP will be clinically supervised by the psychiatrist ad will administratively report to the ACT Team Leader.

The Psychiatric Care Provider (psychiatrist, nurse practitioner, physician assistant) is a member of an interdisciplinary team that ensures service availability 24 hours a day, 7 days per week to persons determined to be so debilitated with their illness and/or unpredictable that they cannot be met effectively at a less restrictive level of care with available community services. The ACTT Psychiatric Care Provider will work closely with the interdisciplinary team and collaboratively with the Team Lead and psychiatrist to include clinical supervision, management of mental health medications, clarification of mental health diagnosis on an on-going basis, assisting with linkage to primary care providers for general medical needs, assisting with crisis intervention, providing supports to currently involved social service agencies, and providing recommendations for linkage to needed social services agencies.

Psychiatric Care Providers are full members of the team and shall perform the following activities in the community in support of both the beneficiaries (clients) and the ACT team staff:

  • typically sees the beneficiaries for the assessment and treatment of the beneficiary's symptoms and response to medication including side effects. Frequency will vary for each beneficiary, with the majority seen within 4 to 6 weeks of last appointment, with many of the contacts being in the community. Less frequent visits should only occur when there are unusual circumstances present (for example, when the beneficiary has been difficult to find);
  • collaborates with the team leader in sharing overall clinical responsibility for monitoring beneficiary treatment and clinical supervision to the team;
  • actively collaborates with nurses to develop and implement medication administration policies and procedures as well as oversee the medical care of NC Division of Medical Assistance Medicaid and Health Choice Assertive Community Treatment (ACT) Clinical Coverage Policy No: 8A-1 Program Effective Date: December 1, 2015 16D8 24 beneficiaries that include regular screenings for medical conditions and assessment of wellness and health management;
  • educates non-medical team members on psychiatric & non-psychiatric medications, their side effects, & health-related conditions; provides diagnostic and medication education to beneficiary, with medication decisions based on shared-decision making;
  • regularly participates in daily team meetings and treatment planning meetings; attends daily team meetings in proportion to time allocated on team;
  • provides brief therapy (formal or informal); and
  • provides psychiatric back-up to the program after-hours and weekends. (Note: may be on a rotating basis as long as other psychiatric care providers who share on-call have access to beneficiary's current status and medical records/current medications).
 
Position Requirements

Minimum Qualifications:

The ACT team psychiatric nurse practitioner (PNP) shall be currently licensed as a RN in North Carolina and meet the requirements as specified in 21 NCAC 36.0800, approval and practice parameters for nurse practitioners, with at least three years full-time experience treating individuals with SPMI.

  • Holds a current, unrestricted North Carolina nursing license and is in good standing with their professional board.
  • Holds a current, unencumbered DEA license and is in good standing with federal agency; and
  • Has a current NPI number.
  • Holds a current, unrestricted, state-appropriate driver’s license and valid, unrestricted personal vehicle insurance. Must be able to use personal vehicle for work purposes.
