Calling all LPNs who are interested in a per diem position where you'd only be called to work the day shift!

Under the direction the Registered Nurse the Licensed Practical Nurse (LPN) assumes primary responsibility for providing quality nursing care for individuals served in the PRIDE program. This position requires assessing, planning, implementing and evaluating administration of prescribed medications, tube feedings, and other treatments authorized by parents/guardians or other responsible parties. The LPN works cooperatively with Direct Care Staff to accomplish the goals identified in the Individual Service Plans.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Provide habilitation assistance services, as required by the Individual Support Plan (ISP) for each individual served.
  2. Assist all individuals served towards the achievement of their goals. Participate in direct personal care, scheduled activities and day program activities.
  3. Complete required documentation that supports care and services provided (i.e. ID notes, communication logs and client charts.
  4. Ensure equipment is clean and functioning as well as maintaining a clean safe work environment.
  5. Comply with all federal, state and local regulations and requirements, and company policies and procedures.
  6. Administer medication per prescription orders, document medication sheets, and report adverse reactions.
  7. Assess and triage, monitor seizure activity and determine when to call 911 according to Opportunity Village policy.
  8. Assist with intake and screening prospective clients.
  9. Assist with initial nursing assessments under the guidance of the Registered Nurse.
  10. Receive and obtain medications from family and assure proper and safe storage.
  11. Maintain and record narcotic information conduct inventory of controlled substances.
  12. Observe individuals for condition changes, make note and reports and communicate changes to family/caregivers/physicians.
  13. Assist with serving meals and feeding, monitor meals ensuring that special diets.
  14. Assisting with transporting clients to and from bus.
  15. Administering tube feedings, catheterization, Insulin and oxygen.
  16. Administer small volume nebulizer (SNV) treatments.
  17. Responsible for HIPAA compliance according to Federal and State guidelines.
  18. Assist with restroom needs and hygiene.
  19. Perform other duties and special projects as assigned.

     
    Position Requirements

    SKILLS REQUIRED

    • Ability to communicate effectively with individuals, co-workers and supervisors (verbal and written).
    • Ability to care, respond and work with individuals with developmental disabilities.
    • Must possess social discernment to assess and understand other’s reactions and behaviors.
    • Must encompass professional demeanor.
    • Ability to define problems and draw valid conclusions.
    • Ability to interpret instructions, gain knowledge and have initiative.

    EDUCATION AND EXPERIENCE

    • Required: Current Nevada Licensed Practical Nurse (LPN) license.
    • Required: Current Cardiopulmonary Resuscitation (CPR) certification.
    • Preferred: Three (3) years verifiable work experience working with people with disabilities.
